New Zealand - Order Form - Functional Test Kits

You may order test kits online through this application form. However, as part of our duty of care and practice policy an initial consultation is required to be booked to discuss the test results. The test kit fee listed here does not include the initial consultation fee, which is $295/hour. It is our policy for all Initial Consultations to be prepaid- we will invoice you for this once results are received.No test results are released prior to initial consultation. Results will be emailed once the initial consultation has commenced, along with a customised wellness program based on results and all information provided. Wellness program recommendations provided may include additional fees for (optional) practitioner-only supplementation recommendations, and referral to our integrative health coach or other team of practitioners as deemed appropriate based on your health need

Once you order and pay for your test kit(s), the test kit will be sent to the address you enter on the form. Please follow the instructions carefully regarding collecting your samples and sending back to the lab. Test results can take up to 5 weeks to become available, as the samples are shipped and tested overseas. Once w ereceive your results, we will contact you to book your consultation.

All test kits purchases are non-refundable once your specimen has been received for testing. If you cancel your order prior to your specimen being received for testing a $50 administration fee will apply.
