The OTF Academy Summer Training Program at Lake Nona

Dates: May 27th - July 18th (July 1st - 4th week off)

This is the registration form for the Optimal Training Foundation's Premier Summer Program. Hosted at Lake Nona High School, the program runs Monday through Thursday from May 27th to July 18th, from 8:00 AM to 4:00 PM.

The camp is designed and led by Coach Anton Sealey, with performance training provided by Coaches Dontarian Evans and Demetrius Bronson. It exposes high-level players to top coaches and leadership figures in athletics and education. Guest coaches include Eddie Ababio, Ytalo Manzo Santos, and Alex Bright.

Informed Consent and Acknowledgement

I hereby give my approval for my child's participation in any and all activities prepared by the Optimal Training Foundation Soccer Camp during the selected camp.

As a condition of my child's participation in the Optimal Training Foundation Soccer Camp, I understand and agree to the following:

• I assume all risks and hazards incidental to the conduct of the activities.
• I release, absolve and hold harmless Optimal Training Foundation Soccer Camp and all its respective officers, agents, and representatives from any and all liability for injuries to my child arising out of traveling to, participating in, or returning from selected camp sessions.
• In case of injury to my child, I waive all claims against Optimal Training Foundation Soccer Camp, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event.
• I understand that there is a risk of being injured that is inherent in all sports activities, including soccer. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

BY AGREEING ABOVE AND SUBMITTING THIS FORM, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT IS EQUIVALENT TO AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
