If you are submitting a medical question, it must be for yourself or for someone very close to you. If it is for another adult, you must have their consent to inquire of astrology about their condition. If the question is for a child, you must be the adult caregiver. It is against my ethics to read charts for people outside of these criteria.

Your email address or phone number (whichever is your preferred method of contact). I will not share this with anyone! I just need to be able to inform you if you win.

If you are submitting a medical question, please provide the date and time of birth of the patient, along with the location where they were born (city, state/province, country).
If you are submitting a medical question, do you know the date and time that the illness became serious enough that you realized it was a problem? (For example, when you got so sick you had to lie down or leave work.). It is fine if you don't know this— it just affects what type of chart I will pull.
