Arkansas Exquisite Entertainment Waiver & Release Form

This form should ONLY be signed for our Roller Wheel 360, Bubble Ball Blast, and Extreme Bubble Ball Soccer services.
The participant and/or guardian of a participant(s) agrees to the following:
- I am 18 years old and/or have been given permission to sign on behalf of a minor under 18 years old.
- By renting any equipment or services provided by Arkansas Exquisite Entertainment, the participants and/or guardian of the participant agrees to hold harmless Arkansas Exquisite Entertainment, its owners, and employees from any claims or judgments for damages and expenses arising from the actions of the renter, their guests, or any other persons present at the event during setup, during the event, and/or take-down after the event concludes.
- Arkansas Exquisite Entertainment is not responsible for any damage requiring repairs or additional cleaning beyond normal wear and tear, including theft or damage to equipment and supplies, as well as any soiling of equipment. This includes, but is not limited to, vomit, blood, colored or sugary drinks and liquids, or excessive mud. I assume responsibility if I cause and/or am a part of contributing to any damage caused to any persons and/or equipment.
- I can only participate in any event with Arkansas Exquisite Entertainment if I am medically and physically well.
- I agree to receive medical care and/or transportation when deemed necessary in the event of an injury. Arkansas Exquisite Entertainment is released from any liability and payments regarding medical care.
- I understand and assume all risk from attending and/or participating in an event by Arkansas Exquisite Entertainment. Arkansas Exquisite Entertainment is not liable for any injuries sustained or caused by any persons and/or animals in attendance.
- I agree to follow all local and state laws and ordinances. Failure to do so can result in immediate termination of event with no refund.
- I agree that any event coordinator with Arkansas Exquisite Entertainment reserves the rights to deny participation and/or ask a participant/attendee to be removed from any event if participant/attendee is causing harm to self, other persons, equipment, and/or any other reason that negatively impacts an Arkansas Exquisite Entertainment Event. Failure to rectify can result in immediate termination of event with no refund.
I acknowledge that photographs or videos of myself/my child may be taken during the activity and can be used for promotional purposes. If I do not agree to photographs or videos being captured of myself/my child, I agree to notify the event coordinator(s) upon arrival to an Arkansas Exquisite Entertainment Event.
