Medical Release
I/we, the parents or guardians named above, authorize Project Shine, Culture Regeneration Research Society (CRRS Canada) and Vancouver Urban Ministries (VUM) staff or volunteers to sign consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant/s named above.
Use of your child’s pictures
I/we give permission for my children/family photo to be taken and used for CRRS Canada and VUM promotion material.
Field Trip & Outside Venue Permission/Consent
During the Born to Shin Summer Camp (August 6-10, 2024), I, the parent/guardian of the child/children named above, hereby give permission for my child to take part in field trips during this camp.
I understand that my child shall be accompanied by camp counselors during the trip, including while travelling from the departure site to the destination site, and from the destination site to the return site.
I understand that my child is expected to behave responsibly and follow the camp’s discipline code and policies.
I understand that illegal & recreational drugs, alcohol, and weapons are prohibited, and I have discussed this prohibition with my child. I understand that if my child is found in possession of these, the child will be expelled from the trip and sent home.
I/we, named above undertake and agree to indemnify and hold blameless CRRS Canada, VUM and staff and volunteers from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of this camp, as well as of any medical treatment authorized by the supervising individuals representing CRRS Canada and VUM. This consent and authorization is effective only when participating in or travelling to and from events of CRRS Canada and VUM. This includes off-site events, field trips and events requiring physical activity.
Signature
I have read, understood and agree with all of the above, and sign it to cover all of CRRS Canada and Vancouver Urban Ministries’ activities. Activities for the program are effective from the date signed through September 30th, 2024.
