Please complete the following application to be either a participant or vendor at our 32nd Juneteenth Celebration event on Saturday, June 15, 2024. The celebration will take place from 11 a.m. - 4:00 p.m. at Unity Park (301 E. Central Avenue Valdosta, Georgia 31601); SETUP TIME: 9:00 a.m. - 10:30 a.m. Participants will perform throughout the entire celebration and will choose the time of your performance by filling out the participant form. If you would like to be a vendor at our upcoming celebration, you will fill out the vendor application form. 

DISCLAIMER: By filling out this application, I acknowledge that I have read, understand, and agree to all terms and conditions as indicated on this application and vendor agreement by the Southside Library Boosters.
*Additionally, I give the Southside Library Boosters permission to use my image and likeness in any way they deem appropriate in the course of promoting future community service and social engagement events. Vendor booths must be reserved by Friday, May 31, 2024. A full payment of $75.00 is required to reserve a space as a regular vendor/a full payment of $100 is required to reserve a space as a food vendor. Vendors must bring their own tents, tables, racks, chairs, etc., and they must be maintained in the allotted space. NONE of these items will be provided by the Southside Library Boosters. No space will be held without full payment of vendor fee (PayPal is the ONLY form of payment accepted). NO monies will be accepted on the day of the event, all fees MUST be paid in advance! 

Vendor Agreement Form

Please complete the vendor form in its entirety. Vendor space will be assigned on a first come, first serve basis. No vendor shall assign or share a vendor space without the knowledge and written consent of the Southside Library Boosters. Southside Library Boosters reserves the right to restrict vendors for any reason deemed necessary. Vendors must provide completed agreement and full payment of $75.00/food vendor $100 no later than Friday, May 31, 2024, in order to retain their space(s). Once vendors are accepted, all payments are final, and no refunds will be disbursed. Vendor set-up and check-in will be from 9:00 a.m. - 10:30 a.m. on Saturday, June 15, 2024. NO items may be left unattended.

(NOTE: If you are a participant in our program, you are simply volunteering your time and efforts in which we sincerely appreciate)!
