    Sinks are important for hand hygiene. They must be easily accessible and be equipped with SOAP and PAPER TOWELS at all times.

  • Operational Commitment for Skin Invasive Personal Services

    The following questions are directly related to the operation of your temporary personal services booth. Please answer every question. Choose N/A for any questions that are not applicable.
  • Instruments & Supplies

  • Are you using disposable, pre-sterilized, single use critical equipment such as: tattoo cartridges, needle bars, piercing needles, jewelry, or syringes?*
  • Are you using single use (disposable) barriers to cover client contact surfaces and equipment such as: barrier film, dental bibs, drape sheets, machine/bottle bags, or cord sleeves?*
  • Will you be using non-sterile, single use items such as elastic bands and corks?*
  • Will any equipment be reprocessed on-site (at the event)?*
  • IMPORTANT NOTE: Personal services workers must be able to describe the classification for each piece of equipment they use. 

    • Critical - equipment that punctures skin or enters sterile tissues of the body.
    • Semi-Critical - equipment that comes into contact with mucous membranes or non-intact skin but is not intended to puncture either of them.
    • Non-Critical - equipment that only touches hair or intact skin.

  • Definition

    A cosmetic product is defined as a substance or mixture that is manufactured, sold and represented for use by application on or injection into the body for the purpose of:

    • enhancing, preserving or altering the appearance of the skin, hair, nails or teeth,       or
    • cleansing the skin, hair, nails or teeth,

    but does not include an antiseptic product applied for the purpose of reducing, inactivating, or preventing the growth of micro-organisms on the skin before a personal service is performed.

  • Will all cosmetic products be clearly labeled with information about product identity and manufacturer?
  • Will cosmetic products be accompanied with a complete list of ingredients and manufacturer's instructions for use?
  • Will workers ensure that cosmetics are used in a manner that is consistent with the manufacturer's instructions for use, including expiry and use by dates?
  • IMPORTANT NOTE: cosmetics and other supplies must be stored in a manner that prevents contamination.

    • client name and contact information
    • date and description of the service provided
    • name of the worker that worked with the client.

  • Will sterility assurance documentation of all critical equipment used during the service be kept with the client's records?
  • Will a waiver or consent form be provided for invasive procedures (and retained by the vendor or organizer for their records)?

  • Note: the type of flooring required is dependent on the duration of the event, the location of the event, and the type(s) of personal services being performed. Generally speaking, floors must be smooth, cleanable and non-absorbant as well as free from physical hazards that would cause individuals to trip, slip or fall. Reccomended materials include plywood, vinyl tile, vinyl plank, linoleum or concrete. Other options may be discussed with the Public Health Inspector assigned to the event.

    Please ensure that the Zone that you selected at the start of the form is correct to avoid processing delays. 

    IMPORTANT NOTE: 

    1. Applications must be received no later than 30 days prior to an event, AND
    2. Edits to an application must be received no later than 14 days prior to an event.

    Notifications (and changes) received later than the above may not be reviewed or approved by the department.
