Apprentice Raise Request
This form is to be completed by the following classifications only:
- Inside Wireman Apprentice
- Telecommunications Apprentice
- Advanced Placement Apprentice
To receive a raise all of the following conditions must be met:
- You must have at least an 80% grade average.
- You must have the minimum hours required.
- You must not have had any late work reports in the previous semester.
If you believe you have met all of the conditions above, please complete the following form and someone from the apprenticeship office will contact you. All raises will be sent effective the next available pay period for your contractor.
