*** REMINDERS ***

1. A $25 NON-REFUNDABLE deposit is due at the time of registration to reserve your space.

2. We have a hard registration stop date to support administration and teachers with final preparations. Your invoice must be paid in FULL by Monday, August 5th, 2024. There are no refunds past 08/05/2024, under any curriculumstances.

3. This is just the co-op sign-offs. Failure to complete this form in addition to completing your registration, means that your registration is NOT complete. 

4. If you are registering more than 4 students, you will need to complete a second sign-off form.

 5. After you complete your registration form and sign-offs, please take a look at your class schedule in Homeschool Life. Make sure you have selected all the classes of your choosing and that you don't have any unintentional gaps in your block schedule.

6. If there are any problems that we notice with your registration, we will email you. it is important that you respond as soon as possible. The longer you wait to reply to any emails, the more chance there is of classes filling up leaving you no room to make any changes.

7. Once your registration is complete and your invoice is paid in FULL, you will then be emailed a link to join our BAND app. All communication will take place within the app once you join.

8. Mark your calendar for our MANDATORY parent/student orientation and Picture Day on Friday, August 16th, 2024 from 10am to 1pm. 
It will only take about 30/40 minutes to complete all of your action items.

I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TOHOLD HARMLESS East Lake Community Church, Homeschool Collaborators United, HCU, HCU Co-Op, HCU Hybrid Academy, HCU Co-Op & Hybrid Academy, India Horton, HCU, East Lake Community Church administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "Releases" herein) from all liability, claims, demands, losses, or damages on account caused or alleged to be caused in whole or in part by (a) my acts or omissions in organizing or conducting the Activity and (b) the negligence of the Releases or otherwise, including negligent rescue operations or emergency medical treatment, and further agree that if, despite this release, anyone makes a claim against any of the Releasees named above, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS EACH OF THE RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS LIABILITY, DAMAGE, AS WELL AS ANY AND ALL OTHER COST.

HCU Flight School information

 
Welcome to the opportunity of a lifetime! HCU Co-Op & Hybrid Academy has partnered with S&S Aviation to provide an epic Flight School for our students 14 and up! Whether you are looking for a High School elective, time in the air, or looking to receive your Private Pilot License we have the path for you.

 
When registering, please take into consideration that there are no refunds, or prorates under any circumstances past August 5th,2024. We understand this is a high-ticket item, so we only want those who are serious about the class and opportunity.  

 
There is a minimum of 6 students required to keep the class alive. Failure to meet the minimum is the only occasion where HCU will issue refunds. Refunds will be issued on Monday, August 12th, 2024.

Flight school classes are mandatory, and the flying time is optional.

Flying time starts September 11th and will then run every other week.

COVID

The World Health Organization has declared the novel Coronavirus (COVID-19) a worldwide pandemic. Due to its capacity to transmit from person to person through respiratory droplets, the government has set recommendations & guidelines.

I agree to indemnify, defend, and hold harmless the Organization from and against any and all costs, expenses, damages, lawsuits, and/or liabilities or claims arising whether directly or indirectly from or related to any and all claims made by or against any of the released party due to injury, loss, or death from or related to COVID-19.

MEDIA RELEASE

Photographs and videos are routinely taken at co-op events & classes. I release the use of any images taken at this event for the purposes of photographing, video recording, or streaming of the event for promoting the co-op. With my signature below, I agree that images that are taken at this event by or on behalf of the event organizer may be used without compensation or additional permission, and can be used in any way.

I release and allow images, and videos to be utilized for advertising and social media purposes for the HCU co-op for the foreseeable future.

CODE OF CONDUCT AND CODE OF ETHICS

1. I agree to be bound by any Co-Op Code of Conduct, Code of Ethics, policies, and procedures, and I understand that my participation in this event is contingent upon my adherence to the Code of Conduct and Code of Ethics and safety guidelines. 

2. I hereby acknowledge that I will try my very best not to induce, bring about, prompt, or cause any unnecessary negativity, or drama, and there is absolutely no gossiping aloud. If there is ever a situation that I feel I need assistance in handling, I will IMMEDIATELY notify a person in leadership.

3. I agree to go to the BAND files and Read the HCU Co-Op & Hybrid Academy Handbook.

4. I understand and agree that HCU Co-op & Hybrid Academy policies and procedures may change at any point in time as the co-op sees fit for the betterment of the organization.
