    Thank you for trusting me with the opportunity to care for your child. My goal is to provide a safe, supportive, and engaging environment where your child feels comfortable, respected, and encouraged to be themselves while giving caregivers time to rest, recharge, or attend to other responsibilities. This questionnaire helps me get to know your child, including their strengths, interests, routines, communication style, sensory preferences, medical needs, and any supports that help them feel successful. The more information you provide, the better I can personalize your child's care and create a positive experience.

  • Service Rate (Be aware that I have a full-time job and day-time availability is limited during the week Monday- Thursday. On Fridays , there is a little more flexibility. Most weekend and evenings are available ) Drop off : 7 am - Pick up times : no later than 9 pm

My goal is to provide a safe, supportive, structured, and engaging environment where children can learn, play, build confidence, and enjoy meaningful activities while caregivers receive time to rest or attend to personal responsibilities within the provider's home.

    For the safety of all participants, the address of the provider's home will be shared only with families who have completed the required intake forms and have a confirmed reservation.

    Rates, Scheduling & Payment Policy
    The respite care rates are listed above in this agreement.

    Payment in full is due before or at the beginning of each scheduled respite care visit.

    Before scheduling a session, please contact me to verify that your requested date and time are available. A session is not considered reserved until you have received confirmation from the provider.

    An invoice or receipt will be provided via email, text message, or as a written copy, depending on your preference.

    Failure to submit payment by the beginning of the scheduled visit may result in cancellation or rescheduling of the session unless prior arrangements have been made

    I am a Certified Occupational Therapy Assistant (COTA) with experience supporting children in the areas of fine motor development, handwriting readiness, visual-motor integration, sensory-based activities, self-care skills, and school readiness. The services provided  are non-medical respite care services and are NOT occupational therapy services. Participation  does not establish an occupational therapy provider-patient relationship. 

    Activities may incorporate developmental play, fine motor experiences, sensory-friendly activities, crafts, games, movement, and school readiness activities solely for enrichment and engagement.

    If your child requires occupational therapy, speech therapy, physical therapy, behavioral therapy, or other healthcare services, please consult the appropriate licensed healthcare provider. I can not diagnose or provide services outside my scope of practice.

    About me: My name is Shaquita Cloud, COTA/L, CPhT . I am a Licensed  Certified Occupational Therapy Assistant (COTA/L) with experience in pediatric development. I have been practicing since 2020, beginning my career in Early Intervention before transitioning to my current role as a school-based pediatric Occupational Therapy Assistant. Throughout my career, I have worked with children with a variety of developmental needs, including autism, developmental delays, sensory processing differences, fine motor challenges, and school readiness skills. My experience allows me to create engaging, individualized activities that support each child's strengths and interests.
