I understand that the purpose of this form is to initiate a tattoo consultation. The artist will contact me upon receiving this form to schedule a tattoo appointment, making the best effort to accommodate my preferred date and time.

I understand that a $100 deposit is required prior to the tattoo consultation. This deposit will be applied toward the total cost of your tattoo if the appointment takes place as scheduled. If you fail to attend your appointment without providing at least 48 hours' notice, the deposit will be forfeited. Should you wish to reschedule after a missed appointment, a new $100 deposit will be required.
