Authorization and Agreement:

Authorization to Surrender: I, the undersigned, authorize the surrender of ownership of the above-named cat to Solano Cat Sanctuary.

Release of Information: I understand that the information provided on this form may be used by Solano Cat Sanctuary for the purpose of historical documentation and potential adopters.

Health Records Release: I authorize the release of any available health records and veterinary information related to the cat to Solano Cat Sanctuary.

Alteration: I understand that any cat residing at Solano Cat Santuary must be altered (spay/neuter surgery). Any cat needing alteration surgery will be scheduled at the next available appointment with a Low Cost Clinic and will most likely recieve an ear notch.

Liability: I understand that although Solano Cat Sanctuary will make every reasonable effort to provide basic needs for the cat's well-being, including vaccinations and veterinary visits to resolve illnesses, there is no guarantee to avoid sudden health issues, pre-existing conditions, or other situations which my cause unexpected death to the cat.
