Phi Upsilon Omicron 2024 Conclave Registration Form

Please use this form to register for Phi U Conclave 2024 which will be held in Menomonie, Wisconsin, Thursday, October 3rd - Saturday, October 5th. You will have the option to preview, print, and then submit the form once completed. If you wish to pay by check, please select this payment type and follow the directions per the form below. Regardless of payment type, you will still electronically submit your registration form. If you wish to pay by credit card, a $15.00 non-refundable convenience fee will be added to your total. A green check mark will appear once your registration has been successfully submitted. You will also receive an email confirming your registration. More information about Conclave and the program is available at www.phiu.org/conclave. We can't wait to see you!

Registration Information:

• Registration Deadline: Friday, September 6, 2024 (If postmarked/submitted after this date, add $50 to registration fee.)
• One registration form is required for each Conclave attendee.
• Please check with your chapter to confirm delegate information prior to registering.
  
• No refunds after September 13, 2024 (Subject to a $25 cancellation fee.)
• Make checks payable to: Phi Upsilon Omicron Tau Chapter (Include attendee name(s) on memo line of check.)

Note: ONE collegiate delegate for each collegiate chapter receives complimentary registration from the Phi Upsilon Omicron National Office. ONE alumni delegate for each alumni chapter receives a $100 reduced rate registration. All delegates must still complete the registration form and submit electronically. If paying by check, please include a copy of your registration and mail to Dr. Lindsay Barnhart. Collegiate delegate does NOT need to include a check for a registration fee; Alumni delegate, include a check for the reduced registration fee as indicated below with completed form. All others send payment with registration.

Hold Harmless Participation Clause:

Phi Upsilon Omicron is committed to providing a safe and healthy conference atmosphere for our in-person attendees in partnership with the hotel. The latest safety guidelines and protocols will be followed on property.

By registering for this event, you are acknowledging that an inherent risk of exposure to infectious, communicable diseases exists in any public space where people are present, even with health and safety measures in place. By attending the event, you voluntarily assume all risks related to your attendance and participation in the event, including exposure to infectious, communicable diseases and agree to release and hold harmless Phi Upsilon Omicron and its members, conference attendees, staff, and vendors from any and all claims, demands, and causes of action arising out of or relating to your attendance or participation in the event, including but not limited to release from any liability related to any illness, injury, or death resulting from your attendance at the event. You further agree that, by registering, if the event is canceled or postponed, Phi Upsilon Omicron is not liable for any hotel and/or travel expenses that you may have incurred in connection with the event.
