  • IN CASE OF EMERGENCY: It is the policy of Kai Oni Canoe Club to contact the family of the paddler regarding medical treatment if the paddler is injured or becomes ill. I give consent to have me/my child transported for treatment to the closest source of medical aid at the discretion of the coach or person in charge. I agree to assume all responsibility for medical and dental care of myself/my child/ward while I/they are involved in the activities of Kai Oni Canoe Club. I authorize the City and County of Honolulu Emergency units to administer appropriate treatment.

  • This release is executed on  Pick a Date   , between KAI ONI CANOE & ATHLETIC CLUB, INC. of Honolulu, Hawaii hereafter referred to as "CLUB" and , in behalf of myself/my minor child      of Honolulu, Hawaii.

    I         "Releaser", being over the age of 18 years, in consideration of being permitted to participate in the sport of canoe paddling, with the CLUB, do hereby release and hold harmless the CLUB from any and every claim, demand, action or right of action, of whatever kind or nature arising from or by reason of any injury to me known or unknown, death or property damage resulting from any accident which connects with canoe paddling (including social activities), whether by negligence or not.

    I further release all members and coaches from any claim whatsoever I may have on account first aid, treatment or service rendered to my myself/minor child (referred to above) during his/her participation in this sport.

    This release contains the entire agreement between TEAM and I and the terms of the release are a contract.

    I have read this release, I understand it, and I sign it as my own free act. I understand that before I sign, I may have my attorney review this release and he or she can explain in detail the full extent of the legal rights I am giving up.

    IN WITNESS WHEREOF, "Releaser" has executed this release in Honolulu, Hawaii, the day and year first above written.

  • Photo Release Waiver

  • I grant Kai Oni Canoe and Athletics Club, Inc., its representatives and employees permission to record on photography, film and or video, pictures of my participation in connection with all club sponsored activities.

     

    I authorize Kai Oni Canoe and Athletics Club, Inc., its assignees and transferees to copyright, use and publish the same in print or electronically.

     

    I agree that Kai Oni Canoe and Athletics Club, Inc., may use such photographs, film and or video of me with or without my name for any lawful purpose, including for example, such as purposes as publicity, illustration, advertising, and web content.

     

    I further agree that such use shall be without payment of fees, royalties, special credit or other compensation.

     

    I have read and understand the above and by signing below attest to such.
