WSA FINANCIAL ASSISTANCE INFORMATION
WSA offers financial assistance with monthly installments. Financial assistance does not cover associated "team fees" (i.e. tournament fees, travel expenses, etc) or Uniforms or Annual Fall & Spring Registration Fee.

Financial Assistance Screening process has been moved to ONLINE ONLY.

Upon completing this form you will receive communication from a club administrator on approval of request, or a differing approval amount, or further information or documentation required.

Please note that if you fail to provide the required documents in a timely manner, your application may not be processed, and you may not be considered for financial assistance for the 2024-2025 season. Note that required documents should be uploaded to this application.

The approval will be based on available funds, and objective review of the information in the application. All approvals are subject to processes determined by the WSA Executive Council and administered by the club's administrative staff.

DEADLINE TO APPLY IS MAY 20, 2024 for the 2024-2025 seasonal year.

After completing this application for Financial Assistance you will receive an email confirmation of the submission which will contain a discount code you can use to waive your intial fees. You should register your child within 24 hours of completing this application. Registering your child places your child on the team's roster and is a required step.

Once your financial assistance application is reviewed and approved, you will receive notice and be asked to submit a FINANCIAL ASSISTANCE ACCEPTANCE FORM. Please be sure to complete the ACCEPTANCE FORM as soon as possible, so that an administrator may revise your invoicing structure to reflect the discounted and deferred payment. At this stage the payment plan will be active, and your child will be added to the team's roster.

Reminder that applying by the deadline is important because it provides a 2-week window prior to the June 1st installment.

If you miss the deadline, then the June 1st payment will be withdrawn through the auto-debit process. However, if subsequent to the withdrawal, and before you receive notification of your financial assistance award, and agree to the financial assistance awarded, WSA will allow applicants to request to withdraw their registration.

About Kinsey Wright

WSA Soccer financial assistance applicants who receive financial assistance will be alerted when this assistance is allocated from the Kinsey Wright Beloved Fund. Each recipient will be provided with a recipient letter, which will honor Kinsey's impact on our world, identifying these beloved traits of Kinsey, that Kinsey lived, and that Kinsey's family and friends cherish:
- Faith. Kinsey had custom cleats, with her choice of words on each. One shoe said “Trust”, and the other said “God”. From the moment she accepted Jesus to her last day on this earth, that is exactly what Kinsey did - she Trusted God.
- Driven. Kinsey loved soccer and had goals within the game. She worked to reach them. No one did more outside of practice than Kinsey.
- Resolute. Kinsey dealt with setbacks. In her first high school game, she started varsity as a freshman. And in that same game, she tore her ACL. It normally takes 9-12 months to fully return from his injury. Kinsey was on track to be back in 7 months.
- Connected. Kinsey loved soccer, but she especially loved doing it with her team. Kinsey was awesome at her craft so everyone in Tulsa wanted her - but what she wanted was to do soccer with her team.
- Kind, Funny and Caring. Everyone who knows her has a story about her heart, her humor, and her humanity.
- Beloved. More than anything, Kinsey was loved. Loved by her team and teammates, loved as a sister, a friend and a daughter.
  The Kinsey Wright Fund will magnify, extend, and sustain the impact, and the legacy of the beloved person, and life, of Kinsey Wright.
Each Recipient will be invited to and requested to attend a dinner, provided by WSA Soccer, to meet Kinsey's family, coach, teammates, to help continue to proclaim Kinsey's message, and magnify Kinsey's Life and Legacy.

BY SIGNING BELOW, I hereby consent to West Side Alliance Soccer Club registering me with US Club Soccer or the league platform sanctioning body to which my team is participating. I understand that I may be registered to only one US Club Soccer, USYSA or any sanctioning member club at any time. I understand this is a one-year commitment.

PAYMENT PLAN AGREEMENT

BY SIGNING BELOW, I hereby consent and agree to enter into a payment plan with West Side Alliance Soccer Club for the 2024-2025 seasonal year. I understand this is a one-year commitment. I have reviewed the financial commitment and agree to the payment of this amount in full per posted terms on payment commitment, refund policy, and financial assistance application process.

COMMITMENT UNDERSTANDING
I understand that this application signifies my intent to register with WSA Soccer, and thus does commit my child to WSA Soccer for the coming year. If financial assistance terms are not met by WSA this can void my commitment. And I am committed to working through a process to reach an agreed to financial commitment based on my financial need, and willingness to contribute toward the overall operating expenses of my child's soccer team.

If you are requesting less than 25% assistance, you MUST provide two (2) of the documents listed below with your application.

If you are requesting 26% - 50% assistance, your 2020 or 2021 Federal Tax Return is required AND two (2) additional supporting documents from the list below:
• 2022 or 2023 Federal tax return, AND
• One form of supporting documentation of Income;
o Last two (2) consecutive pay stubs for each wage earner, or Social Security Annual Benefits Statement, or Unemployment Benefits Statement
• One or more supporting documents of Assistance;
o Food stamps, Sooner Care Insurance, Oklahoma Department of Social Services or other State/Federal assistance program, may be submitted to fulfill the two document requirement.
• Below, please describe any special circumstances that you wish to be considered by the Financial Assistance Committee. If submitting, attach to this form or use the dialog box at the bottom of this form.

FINANCIAL ASSISTANCE AGREEMENTS
By checking this box I hereby certify that the information provided is accurate and truthful. I understand that incomplete applications will NOT be processed or considered by the Financial Assistance Committee. I acknowledge that any amount of Financial Assistance granted by West Side Alliance Soccer Club shall require the use of Automated Clearing House (ACH) payments for the collection of the agreed upon Monthly Installments; I shall provide my banking information at the time of Registration to the Club for this purpose.*

Parents further understand that WSA is a volunteer driven organization, and are willing, where possible, to serve as volunteers throughout the year to assist the program.*

I understand that accepting this financial aid is also an agreement of commitment to West Side Alliance S.C. All information provided above is accurate and correct to the best of my knowledge. I understand that WSA will do its best to provide and meet all financial assistance requests, but application is not a "guarantee" of assistance. I further understand that "team fees" are not considered in the financial assistance program, and I am willing to accept responsibility for "team fees" (associated with entry fees, travel, referee fees, etc).*

If I transfer to another team or do not fulfill my year long commitment, the WSA Board may hold me responsible for the full pre-discounted amount, or portions of the full amounts of financial assistance awarded. WSA expects, as per the Player Contract, that players are honest and forthright in any discussions they have with other clubs or teams.*
