1. In consideration of the Williston Animal Group (WAG) permitting me to serve as a volunteer to this organization the undersigned does herby release, remise and forever discharge, indemnify and agree to HOLD HARMLESS, WAIVE, DISCHARGE and COVENANT NOT TO SUE WAG, their officers, board of directors, volunteers, employees and members, hereinafter referred to as RELEASEES, from any claim, demand or cause an action whether now in existence, or hereafter arising for any loss of personal property, injury or death, arising out of resulting from, caused by contributed to in whole or in any part by any action or failure to act, negligence, breach of contract, or other misconduct on the part of WAG, its officers, board of directors, volunteers, employees and members.
2. Further, I recognize and acknowledge that certain risks of harm are or may be inherent in the various activities involved in serving as a dog rescue volunteer. I am aware that some dogs can pose dangers to people and other animals. Such dangers may include, by way of example and not limitation, dogs can bite, trip, knock down, and fight or otherwise spread germs or bacteria. Additionally, by way of example and not limitation, indoor and outdoor surfaces can become slippery, equipment cancause slips,trips and falls and other persons may fail to control their dogs. Activities involving dogs may result in injury or damage to property.
3. It is my express intent that this Release and Hold Harmless Agreement (hereinafter “RELEASE”) shall bind the members of the family if I am alive and my heirs, assigns and personal representative, if I am not alive. It shall be deemed as a RELEASE, WAIVER, DISCHARGE, AND COVENANT NOT TO SUE the above mentioned RELEASEES. Hereby further agree that this RELEASE shall be construed in accordance with the laws of the State of Florida.
4. I UNDERSTAND THAT WAG WILL NOT BE RESPONSIBLE FOR ANY MEDICAL COSTS ASSOCIATED WITH ANY INJURY OR ILLNESS THAT I MAY SUSTAIN AS A RESULT OF MY VOLUNTEER ACTIVITIES.
IN SIGNING OR SUBMITTING THIS RELEASE, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing RELEASE, understand it and sign OR submit it voluntarily as my own free act and deed; no oral representations, statements or inducements, apart from the foregoing written agreement, have been made; I am at lease eighteen (18) years of age or am the legal guardian or parent of a person under the age of 18 and fully competent, and I execute this RELEASE for full, adequate, and complete consideration fully intending to be bound by the same.
5. Circumstances may arise where the Williston Animal Group finds it necessary to terminate your active volunteer status. Such action may be taken at any time, without prior notice, and for reasons deemed appropriate by the organization's Board of Directors. We value your contributions, but reserve the right to make decisions that ensure the best interests of our organization and the animals we serve.
PLEASE READ THE FOLLOWING AS REGARDS HUMANE EUTHANASIA AND WILLISTON ANIMAL GROUP POLICY:
6. I understand that Humane Euthanasia is a possible outcome for dogs under the care and control of Williston Animal Group, which encompases both dogs residing at the kennel facility and in foster homes. Williston Animal Group defines "No Kill" as a facility that does not euthanize otherwise adoptable dogs in order to reduce numbers or make space for other dogs. The Board of Directors are the sole responsible party for making these decisions. There are many factors that are considered when making decisions regarding humane euthanasia, including, but not limited to: the overall health and quality of life of the dog, the risk or possible likelihood of a dog harming a human or animal, behavior or medical conditions that create extremely difficult or expensive management, behavior or medical conditions that can not be treated and/or quality of life while confined in a kennel environment. The board of Directors make these decisions carefully, weighing all possible options known to them at the time. A majority vote of the Board is needed before humane euthanasia is decided. Humane Euthanasia is performed by a liscensed Veterinarian at our direction. Volunteers are free to ask details of the Boards' decision from any Board member and are free to share that conversation or refrain from sharing with other active Volunteers.
