By signing this waiver form, I acknowledge and confirm the following:*

Check-in is at 4:00pm and check-out is at 10:30am. Early check-in and late check-out is available by texting us at (706)809-8570. Minimum fee of $25 may apply.All reservations must be secured with a minimum security deposit of $150 by a major credit card or debit card. (If you booked with a third party, such as Booking.com or Expedia.com, please contact the hotel at (706)878-2689 ext. 5 to update your card information for this deposit.)I understand that I will not be able to check-in unless the security deposit has been paid.I understand that any theft or damages will be charged to my credit card or debit card on file.I authorize the Heidi Motel to charge the card provided.I understand that the smart lock code will be given to me day of check-in via phone, text, or email by 4:00pm. I understand that it is my responsibility to make sure I have the smart lock code by 4:00pm. Even if I am arriving late.I understand that I must vacate the property by check-out time and that staying past check-out time interferes with property cleaning and violates the Heidi Motel terms and conditions. I understand this may result in overstay fees.I understand that there will be a $25 per night per pet cleaning fee.I understand that if I have a pet and I do not report this pet, I will be charged a minimum fee of $150.I understand that all rooms are Non-Smoking and smoking in a room will result in a minimum fee of $150.I agree with the following policies/statements when booking a hot tub room: 1.) I should NOT use hot tub alone. 2.) I should limit myself to 15 minutes per hot tub session. 3.) I should NOT use hot tub if I am on certain medications or under the influence of drugs or alcohol. 4.) I should consult my physician before using hot tub if I am pregnant or elderly. 5.) I will NOT dive into hot tub or swim under water in hot tub. 6.) I will NOT have glass or food in hot tub. 7.) I will not have electrical appliances, devices, or cords around hot tub. 8.) I am aware that the floor could be slippery when exiting hot tub and I need to be cautious. 9.) I will not use bubble bath, shampoo, or any kind of soap or oils in hot tub. 10.) I am aware that breaking any of these policies will result in a $150 minimum charge and may result in immediate eviction.I understand that if I have booked a hot tub room, they are drained and filled the day of check-in. We prefer a 2-night stay in hot tub rooms. We cannot guarantee the temperature of the hot tub on 1-night rentals to be fully heated by time of check-in.I understand that if I need housekeeping that I need to text (706)809-8570. I also understand that this text needs to be sent between 9am and 1pm. We will do out best to accommodate any texts sent after this time.I understand that no text to the number above means "DO NOT DISTURB". You can also text "DO NOT DISTURB".I understand that there is a $10 per stay per room fee for the cleaning fee.I understand that there is an additional $15 per stay per Hot Tub room fee for the cleaning fee.I understand that there is an additional $15 per stay per Kitchen room fee for the cleaning fee.
