This contract is entered into by and between Taylor Anne Wilkes Duffy (referred to as "Trainer" hereinafter) and Owner First Name Last Name (referred to as "Owner" hereinafter).

This agreement will be in regards the dates selected above, to arrange the services that Owner's pet(s) Dog's name (referred to as "Pet" hereinafter), will receive from the Trainer and arrange the rights and obligations of the parties.

Please include the dates, along with the window for pick up/drop off for boarding below.

If you are unsure of the dates you are welcome to include a rough estimate (e.g. mid June for a weekend, August 7-10th to the best of my knowledge, etc).

Overnight Dog Sitting

$125 per night @ Owner's Home

The service to be provided under this Contract is basic Overnight Dog Sitting at the Owner's home or Trainer's home (upon approval). This includes feeding the Pet in the recommended amount per day, and meeting the dog's physical and mental needs.

In the case of the Pet having extra or specific food, treats, or medicine, it is to be covered by the Owner. The Owner is obliged to provide these products to the Trainer during the agreed upon service. In the event of the Trainer picking up the required products, the Owner agrees to reimburse the Trainer for those items.

Training during the stay is an additional add on along with the overnight stay. Sessions are considered 10-20 minutes working on a skill or mental enrichment. There is no set time of service unless otherwise agreed upon; the sessions are done at the discretion of the Trainer during the day. At the end of the stay, the Owner can expect a email within 24 hours that includes bullet point review of what was worked on, and any additional information regarding the Pet's progress.

The goals should generally be brush up or continued training. Major behavior change goals are not guaranteed over short boarding periods, but are recommended for board and train programs lasting 14 days or more.

Cancellation Policy

Cancellations made 30 days or more in advance of the service date will receive a 100% refund of their deposit. Cancellations made within between 30-15 days in advance of the service will receive a 50% refund of their deposit. Cancellations made within 14 days of the service will not receive a refund.

Any bookings over holidays are not eligable for refund.

The Owner authorizes the Trainer to seek and obtain medical attention and treatment with or without the Owner's express agreement. The Trainer will aim to do what they deem necessary to insure the Pet's health and well being, and is authorized to make decisions regarding the Pet's health while the Pet is in their care. When seeking treatment, the Owner will be contacted in as timely a manner as possible, and be kept aware of the process to the best of the Trainer's ability.

If the provided Veterinarian is closed or inaccessible, the Trainer reserves the right to go to a Veterinary Clinic of their choosing. If the Owner is unable to be reached, the Trainer is authorized to seek and obtain medical attention and treatment for the Pet.

The Owner agrees to pay for/reimburse the cost of any and all veterinary or necessary services that are incurred at this time. Further, the Owner can confirm that their Pet(s) is up to date on all vaccinations and will provide proof during the dates of service. The Owner also confirms that the Pet does not suffer from any life threatening or contagious condition that may be exacerbated by exposure to other pets, or expose other animals to undue risk while in the Trainer's care.

If the Pet(s) bites or injures the Trainer, or any other human or animal, the Owner will be responsible for any resulting injury. If the Pet(s) becomes ill, injured, lost, or dies, the Owner waives and releases the Trainer from any blame arising from such an incident, understanding the Trainer has the Pet's wellbeing first and foremost when the Pet is in the Trainer's care. The Owner shall hold harmless and indemnify the Trainer against any and all costs, expenses, losses, liabilities, and claims arising out of or relating to any acts of the Pet(s) except if arising out of the negligence or intentional misconduct on part of the Trainer.
