Once you click submit a confirmation email will be sent to {studentemailAddress}, {1stParentguardian288}, {1stParentguardian358}, with a copy of the information submitted. You will also be redirected to a second form to request access to a previously recorded informational webinar. This form should be pre-filed with your information, but you will need to click submit again. A link to request the webinar access will also be available in the confirmation email(s) sent.
Once you click submit two emails will be sent to {teacherEmail}:
1. A confirmation with a copy of the information submitted.
2. A nomination notification email that can be forwarded to your students and/or their parents/guardians.
