  • Eligibility 

    Your project should clearly demonstrate how it contributes to the Core20Plus5 priorities, which currently focus on five key health areas:

    1. Maternity – Supporting continuity of care, particularly for women from BAME backgrounds and low-income communities.
    2. Severe Mental Illness (SMI) – Encouraging uptake of annual health checks.
    3. Chronic Respiratory Disease – Increasing vaccinations and support for people with conditions such as COPD.
    4. Early Cancer Diagnosis – Promoting early detection and diagnosis at stages 1 or 2.
    5. Hypertension – Supporting interventions to improve blood pressure control and reduce risk of heart disease and stroke.

    Your project should aim to:

    • Make a meaningful impact on health inequalities within the communities of Airedale and Ferry Fryston.
    • Engage and support residents who are most vulnerable or marginalised in these areas.
    • Contribute to one or more of the five Core20Plus5 health priorities by addressing the wider social factors that influence wellbeing.

    You can apply for a grant to deliver projects in partnership with other organisations if you wish. This joint bid can be for up to £4,000, single organisation bids will be for up to £2,500.

    How we will use your information:
    We will use your contact details to keep you up to date with the progress of your application and if successful to process grant terms and obtain feedback. Your details will be securely stored.

    We use Jotform for our online grant form. Submitted forms will be kept on Jotform for 10 working days (max.) and then transferred onto our secure internal servers. Saved forms will be kept for the length saved, plus 10 working days.
