CONSENT FOR MEDICAL TREATMENT | As the parent or legal guardian of the above-named participant, I hereby give my consent for emergency medical care prescribed by a duly licensed Doctor. This care may be given under whatever conditions are necessary to preserve the life, limb, or well-being of my dependent.
I give permission for my child to:

Participate in Slip-n-Slide Kickball (will get wet)Have their picture taken and shared on church's social media platforms
On the last night of VBS, Wednesday August 6th, we will close with a family night. The entire family is invited and encouraged to attend. There will be water games, food, games, music and more. Kids and teens will get wet so bring a change of clothes. Come enjoy as we celebrate our last night of VBS. It will be an amazing time. Don't miss out!
