I understand that I will be notified in the case of a medical emergency involving my child. In the event that I cannot be reached, I authorize the calling of a doctor and the providing of necessary medical services in the event my child is injured or
becomes ill.

In case of an emergency, and if a family physician cannot be reached, I hereby authorize my child to be treated by Certified Emergency Personnel (i.e. EMT, First Responder, and/ or Physician).

I understand that the Step by Transportation Services will not be responsible for the medical expenses incurred, but that such expenses will be my responsibility as parent/guardian.

Additionally, I agree to keep my child(ren) medical conditions up to date with Step by Step Transportation Services should any changes occur.

I hereby give permission for the transportation of my child to be provided by Step by Step Transportation Services.

Step by Step Transportation Services and its co-organizers are not responsible for lost or damaged personal property. All scheduled events are subject to change. I understand that no fees will be refunded or transferred unless a child is unable to participate due to an accident or illness per physician orders.

** IN ORDER TO ENSURE ALL STUDENTS ARE PICKED UP AND DROPPED IN A TIMELY FASHION, THERE IS ONLY A 2 MINUTE GRACE PERIOD IN WHICH WILL WAIT FOR YOUR STUDENT(S). AFTER 3 LATE WARNINGS, WE WILL TERMINATE YOUR CONTRACT AND SERVICES.**

All payments are due FRIDAY BEFORE THE UPCOMING WEEK. Payments can be made online via our website. Payments not made by SUNDAY will inquire a $15 late fee and could potentially intervene with your child(ren) being on our pick schedule. After 3 late payments, Step by Step Transportation Services reserves the right to terminate contract.

We are a professional bus service that transports students to and from school, home, after school programs & activities, parents job/work sites, a relative’s home or to a destination of your choice (within our pick up and drop off radius) for a weekly rate.

Step by Step Transportation Services are available for morning or afternoon pick and drop off services. A.M. bus service begins at 5:00 a.m. and P.M. service begins at 1:45 p.m. It is our desire to pick up and drop your child off safely to and from all
destinations. 

We certify that our bus service is fun and safe. All of our drivers are background checked with a safe driving record and a love for working with children.

I understand the no refunds or transfers unless due to illness or other unforseen circumstance (proper supporting documentation will be requested)
