I authorize my current and former employers(s), and the educational institutions I have attended, to provide Benton County Fire Protection District #4 ("the District") representatives any information regarding my current and former employment (and educational background, as applicable). I understand that such information may or may not help my application for employment with the district.
I hereby release the district, my current and former employers, any educational institution that l attended, and all other persons and entities from any and all claims, demands, and/or liabilities arising out of or in any way related to this application process - to specifically include (without limitation) disclosing information to the district as part of this application process. I understand that as a condition of employment, a background check will be conducted, and I must pass a pre-employment physical examination. The background includes reference checks, a criminal history and driving record check. Certain positions require drug and alcohol testing, credit checks, fingerprinting, and a psychological examination. l authorize the district to conduct all of the foregoing background checks as well as an investigation as to my skills, experience, character, general reputation, previous employment, work habits, education, training to perform specific job functions, similar background information, as well as verification of all the information contained herein - to the extent determined appropriate in the district's discretion.
I understand that as a condition of employment I must provide the required documentation to prove I am eligible to work in the United States as mandated by the Immigration Reform and Control Act of 1986.
I certify that all statements made in this application are true, complete, and correct to the best of my knowledge, and that any misrepresentation or omission may result in the district rejecting my application, removing my name from consideration, and/or I may be discharged from employment if already hired. I further understand that false and/or misleading information given in my application or during the interview process (or failure to disclose pertinent information) may also result in discharge.
