Medical Emergencies -  In case of emergency, I understand every effort will be made to contact me. In the event that I, nor emergency contact persons, nor the doctor listed above can be reached, I give my consent for the emergency room physician to treat the aforementioned dancer.

Photo Release - I, hereby, grant West Tampa Dance Company permission to use me, my child, any member of my family, or anyone who visits West Tampa Dance Company or their events, to use photography/video for promotional purposes and/or social media and West Tampa Dance Company's website.

Release of Liability - I understand dance can be a strenuous activity and accept responsibility for any injuries my child may incur while participating in any activity at West Tampa Dance Company. West Tampa Dance Company and its instructors, organizers, sponsors, supervisors, volunteers, agents, officers, owners, directors, spectators, participants, and persons involved are not liable for personal injury, or loss of or damage to personal property. West Tampa Dance Company is not responsible for participants leaving the premises.

Assumption of Risk - I, the parent or legal guardian of the applicant listed above, hereby approve the applicant's participation in any and all West Tampa Dance Company rehearsals, classes, performances, programs and activities. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren myself or any member of his/her family whether as a participant or as a spectator (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at West Tampa Dance Company. On my behalf, and on behalf of my children, I hereby waive, release, covenant not to sue, dischage, absolve, indemnify, and agree to hold harmless West Tampa Dance Company, its organizers, sponsors, supervisors, volunteers, agents, officers, owners, instructors, directors, spectators, participants, and persons involved in the operation of West Tampa Dance Company's programs of and from any Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. This includes attorney's fees or any deductibles I may incur.

Waiver of Liability Relating to Coronavirus/COVID-19 - The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is contagious and is believed to spread mainly from person-to- person contact. West Tampa Dance Company will make every effort to keep our studio sanitized and to prevent symptomatic employees and dancers from entering the building. However, COVID-19 has been proven to be asymptomatic in some persons. Therefore, West Tampa Dance Company cannot guarantee that you or your child(ren) will not become infected with COVID-19 or any other unknown virus. I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending West Tampa Dance Company and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I agree that my child and any accompanying persons will not enter West Tampa Dance Company if we exhibit any symptoms of COVID-19. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of West Tampa Dance Company, its instructors, agents, and representatives, whether a COVID-19 or other currently unknown virus infection occurs before, during, or after participation in any West Tampa Dance Company program or class. 

All tuition and fees are due by the 1st of the month. If not received by the 7th of the month, a $15 fee per dancer per class will be added to your next month's invoice. If not received by the 10th of the month, your child will not be allowed in the classroom until the account is made current. There will be a $35.00 fee assessed for any returned payments. First month's tuition is due upon registration and in full each month. Tuition and fees are non-refundable.  Prices are subject to change. Discounts only apply to class tuition. Discounts do not stack. If multiple discounts are applicable, we will take the higher discount off tuition. To drop a class, an Add/Drop Form must be completed and returned to West Tampa Dance Company in person by the 25th of the month to avoid being charged for the dropped class(es) on your next month's invoice. Classes run August through June. For competition team pricing, fees, and policies refer to the Competition Team Manual.

Explanation of Costume Fees - 

• A $100 costume fee per class must be paid in full by December 1st. Costumes will not be ordered until paid in full. 
• We offer two payment plans - Total costume fees may be split into 4 partial payments of $25 per costume added to the September-December monthly invoices, or 2 partial payments of $50 per costume added to the October and November monthly invoices. 
• If you start in/after December 1st, your costume fee must be paid in full at the time of registration. Costumes paid for in/after December are not guaranteed to arrive before picture day.
• The costume fee covers the cost of costume, taxes/fees, shipping, accessories, and tights.
• Costume fees are non-refundable.

Explanation of Recital Fees - 

• The recital fee ranges from $150-$200 and will be split into 4 partial payments added to the January-April monthly invoices.
• Recital fees must be paid in order to participate in recital.
• This fee covers the cost of our recital venue, insurance, crew, recital DVD, recital shirt, and studio gift for your dancer. 
• Recital fees are non-refundable.
