• Pre-registration Form - New Players born in Canada

    This form does NOT imply that your athlete is a confirmed registrant. This form is used to create the Hockey Canada ID number needed to register on the TeamSnap registration form. Please note that it can take up to 10 business days prior to receiving your Hockey Canada ID number and the final email with the registration completion steps. We thank you for your patience.

  • PROOF OF RESIDENCY

    Document – Must be TWO of the following:

    • HOMEOWNERS - 2024 Property Tax Notice (full page is required with the address at the top and bottom visible).  Please note that we cannot accept your Property Assessment
    • Current hydro, gas, water, cable, satellite bill 
    • BC Medical Statement, GST statement, income tax assessment, child tax credit
    • School Registration (confirming child's primary address)

    All proofs of address must be no older than 2 months.  Bills must show the entire full bill page, including name and address.

    Note: We cannot accept telephone/cellular bills, bank or credit card statements as acceptable proofs of address. We can no longer accept lease / rental agreements for tenants.

    FAILURE TO COMPLY WITH SUBMITTING ALL NEW PLAYER DOCUMENTATION IN THE ABOVE MENTIONED FORMAT WILL RESULT IN THE ATHLETE NOT BEING PROVIDED WITH A HOCKEY ID NUMBER.
