Registration is now open for the 3rd Annual Apex for Youth Hoops Over Hate 3v3 Basketball Tournament. We will have three division of play (4th-5th Grade, 6th-8th Grade, & 9th-12th Grade). Each team is allowed to have 4 players.

Report cards for School Year '23-'24 must be sent in to verify age and grade for division.

In 2021, amidst the drastic uptick of Anti-Asian violence, a 13 year old experienced an Anti-Asian attack at PS20 Bowne Playground in Queens. This led to the first ever Hoops Over Hate 3v3 Basketball Tournament in Flushing. Annually, we host this event to create a safe environment for different youth and communities to come together and stand against hate.

*Please note each team member must fill out a waiver in order to participate in the tournament. Be sure to include the same exact team name on your each of your registration forms.*

As a parent or guardian of my minor child, I authorize Apex for Youth to take and use visual/audio images of my child. Visual/audio images are any type of recording, including, but not limited to, photographs, digital images, drawings, renderings, voices, sounds, video recordings, audio clips, accompanying written descriptions, and/or any other type of media now or hereafter known.

Apex for Youth may use and/or authorize the use of my child’s video/audio images and/or my child’s name in any manner or media without notifying nor further examination by me -- such as, but not limited to, websites, newsletters, social media, publications, presentations, exhibitions, broadcasts, advertisements, and/or posters. 

I waive any right to inspect or approve the finished images, prints, or any electronic matter that may be used with them. I agree that all visual/audio images connected therewith are and shall remain the property of Apex for Youth.

I release Apex for Youth, its employees, officers, trustees, administrators, successors, and assigns from any claims, actions, demands, and liabilities of every name and nature which I may ever have in connection with the taking of or use of my child’s visual/audio images and/or my child’s name.

I have read this release before signing it. I understand its content and I voluntarily accept its terms.

Como padre/madre o tutor/a de mi hijo/a menor, autorizo a Apex for Youth a tomar y utilizar imágenes visuales y el audio de mi hijo/a. Las imágenes visuales, o el audio, son cualquier tipo de grabación, incluidas fotografías, imágenes digitales, dibujos, representaciones, voces, sonidos, grabaciones de video, clips de audio, descripciones escritas adjuntas y/o cualquier otro tipo de medio conocido ahora o en el futuro.

Apex for Youth puede usar y/o autorizar el uso de las imágenes de video y el audio de mi hijo/a y/o el nombre de mi hijo/a de cualquier manera o medio sin notificarme, como, entre otros, sitios web, boletines, redes sociales, publicaciones, presentaciones, exposiciones, retransmisiones, anuncios y/o carteles.

Consent to Medical Treatment / 药物治疗同意书 / Consentimiento para Tratamiento Médico
I agree that in the event of an emergency, injury, or illness and in the event that I cannot be contacted, the supervisor or agents of Apex or the internship site may act on my behalf and at my expense in obtaining medical treatment for my child. I hereby give permission to the medical personnel selected by Apex for Youth to secure and administer treatment, including hospitalization, for my child, who is named on this form. I further agree that I release Apex from any and all claims, actions, demands, and liabilities of every name and nature whatsoever that might arise on account of any such medical treatment.

Consent to Participate / 参加许可 / Consentimiento para participar

As the parent or legal guardian of the child listed in this application, I hereby give my full consent and approval for my child to participate in the Apex Youth programs. All the information in this application is true to the best of my knowledge. In addition, I understand that each volunteer that participates in the Apex for Youth programs is an independent volunteer who is not employed by Apex for Youth and that, although Apex for Youth has conducted background checks on each volunteer and gives trainings and some supervision to each volunteer, Apex will not be responsible for any action or inaction of any volunteer (including your child’s volunteer) in connection with any program activity. I also understand that my child’s volunteer may have meetings with my child at times and places of their choosing and that Apex for Youth does not control or supervise any such meeting. I understand and assume any risks associated with my child’s participation in the program and I voluntarily consent to my child’s participation in the Apex for Youth programs. I understand that participation in Apex for Youth programs is entirely voluntary. My child or I may withdraw from Apex for Youth programs without affecting any of my rights.

Liability Release / 意外事故责任豁免同意书 / Exoneración de responsabilidad

In consideration of and in return for services, facilities, and other assistance provided to my child by Apex for Youth and its officers, directors, staff, employees, agents, and representatives, I, on behalf of myself, my child, and my child’s other parent/ guardian(s), if any, hereby absolutely, unconditionally and irrevocably release Apex for Youth and its officers, directors, staff, employees, agents, and representatives from any and all liability, claims and actions that may arise from injury or harm to my child or my child’s property, including, without limitation, any injury or harm to my child or my child’s property that may be caused by the volunteer’s negligence or willful misconduct or result from my child’s participation in the program. I understand that this release covers any and all claims, actions, demands, and liabilities of every name and nature caused entirely or in part by any acts or failures to act by Apex for Youth and its officers, directors, staff, employees, agents, and representatives to the extent that such release is permitted by law. I recognize that this release means I, my child, and my child’s other parent or guardian(s), if any, are giving up, among other things, the right to sue Apex for Youth and its officers, directors, staff, employees, agents, and representatives for any and all injuries, damages, or losses my child may incur while participating in the Apex program. I also understand that this release binds me, my child and my child’s executors, administrators, and assigns. I have read the entire release in this paragraph, fully understand it, and voluntarily agree to be legally bound by it.

En consideración y a cambio de los servicios, instalaciones y otra asistencia brindada a mi hijo/a por Apex for Youth y sus funcionarios, directores, personal, empleados, agentes y representantes, yo, en mi nombre, el de mi hijo/a y el del otro padre/tutor(es), si lo hubiera, por la presente libero absoluta, incondicional e irrevocablemente a Apex for Youth y sus funcionarios, directores, personal, empleados, agentes y representantes de toda responsabilidad, reclamo y acción que pueda surgir de lesiones o daño a mi hijo/a o a la propiedad de mi hijo/a, incluyendo, entre otros, cualquier lesión o daño a mi hijo/a o a la propiedad de mi hijo/a que pueda ser causado por la negligencia o mala conducta intencional del voluntario o como resultado de la participación de mi hijo/a en el programa.
