Please complete this form, once a registration has been submitted your details will be shared with Cure Cancer who will contact your team representative to collect the donation off $1,500 per team.
Before getting started you will need the following information to complete this form:

Please provide your game account ID for Rocket League or Fall Guys

Special Note: for Console / Steam versions of Rocket League:

1. Open Rocket League.
2. From the main menu, head to friends list.
3. Copy down the ID next to the Epic Games logo.

Discord

If you do not have a Discord account, please head to www.discord.gg to create an account.

To find your Discord ID, follow these steps:

1. Navigate to the bottom left of the Discord application/website.
2. Click on the area that shows a display picture/your profile name.
3. A panel will have opened that shows some profile controls.
