    • CONSENT FOR LIMITED QUERIES OF THE FEDERAL MOTOR CARRIER SAFETY ADMINISTRATION (FMCSA): DRUG AND ALCOHOL CLEARINGHOUSE

    • I * hereby provide consent to Howland Logistics to conduct a limited query of the FMCSA Commercial Drivers License Drug and Alcohol Clearinghouse, to determine whether the drug and alcohol violation information about me exists in the Clearinghouse. Terms of consent are for Pre Employment and Annual query's.

      I understand that if the limited query conducted by Howland Logistics indicates that drug or alcohol violation information about me exists in the Clearinghouse, FMCSA will not disclose that information to Howland Logistics without first obtaining specific consent from me.

      I further understand if I refuse to provide consent for Howland Logistics to conduct a limited query of the Clearinghouse, Howland Logistics must prohibit me from performing safety sensitive functions, including a driving a commercial motor vehicle, as required by FMCSA's drug and alcohol program regulations.
