In consideration for participating in a camp program, by signing below, I hereby agree to indemnify and hold harmless Dallas Independent School District and its employees, etc. from any and all claims or demands. I agree to assume all risk and responsibility for my/my child’s participation. In the event of any injury, I understand, acknowledge, and agree that the Dallas ISD, its Board members, officers, and employees shall not be liable for any injury/illness suffered by myself/my child which is incident to and/or associated with preparing for and/or participating in the camp.

I hereby waive, release, and discharge the Dallas ISD, its Board members, officers, and employees from any claim, demand, cost and/or expenses arising out of any injuries, damages or other losses, whether personal or property, sustained by me/my child (or any party to whom I am responsible), by causes of action, suits or judgments of any kind that myself or my guardians, heirs, executors or assigns, etc. may have arising from any activity or services.

I agree that my/my child’s use of facility will be conducted in a manner consistent with the values of Dallas ISD. I (or any party to whom I am responsible) shall comply with all federal, state and local laws with regard to my use of the facility. I agree that I shall be responsible for all damage to the facility that results from my use (or any party to whom I am responsible) of the facility.

Consent for Treatment: I hereby give my consent to have myself/my child treated by emergency medical personnel, a physician, or surgeon, in case of sudden illness or injury while participating in the above activity. It is understood that Dallas ISD will provide no medical insurance for such treatment, and that the cost will be at my expense.

I UNDERSTAND AND ACKNOWLEDGE THAT IN ORDER TO PARTICIPATE IN THIS CAMP, I AGREE TO ASSUME ALL LIABILITY AND RESPONSIBILITY FOR ANY AND ALL POTENTIAL RISKS, INJURIES, OR EVEN DEATH THAT MAY BE ASSOCIATED WITH PARTICIPATION IN SUCH PROGRAMS. 

I acknowledge that I have carefully read this release of liability. 

PHOTO RELEASE WAIVER: J.L. Long Cheer Photo Release Form for Minors. 
I/We hereby grant to J.L. Long Cheer the absolute and irrevocable right and unrestricted permission in respect of any pictures J.L. Long Cheer had taken or will take of me/us or in which I/we may be included with others, to copyright the same; to re-use, publish or republish the same in whole or in part, individually or in conjunction with other photographs, and in conjunction with any printed matter, in any and all media now or hereafter known, for illustration, promotion, art, editorial, advertising and trade, or any other purpose whatsoever; from time to time, or reproductions thereof in color, black and white or otherwise made through any media.

I/We hereby waive any right that I/we may have to inspect or approve the finished products or the advertising copy or printed matter that may be used in connection therewith or the use to which it may be applied. 

I/We hereby release, discharge and agree to save harmless J.L. Long Cheer, its legal representatives or assigns and all persons acting under its permission or authority, from any liability in connection with the use of the photographs as aforesaid or by virtue of any alteration, processing or use thereof in composite form, whether intentional or otherwise, as well as any publication thereof.

I/We understand that the photographs taken by J.L. Long Cheer will be included into stock files. I/We agree that the photographs, the transparencies thereof and the rights to copyright the same, shall be the sole property of J.L. Long Cheer, with full right of lawful disposition in any manner.

I/We hereby grant permission to J.L. Long Cheer to photograph the J.L. Long Cheer student during activities to use the photographs in audio-visual and printed materials without compensation or approval rights.

I agree to allow the named child to participate in the activity identified above and understand all reasonable safety precautions will be taken at all times by Skillman Church of Christ and its agents.  I understand the possibility of unforeseen hazards and know the inherent possibility of risk.  I agree not to hold Skillman Church of Christ, its leaders, employees, and volunteer staff liable for any damages, losses, diseases, or injuries incurred as a result of the child’s participation in this activity.
