As parents of the participant/as a trip chaperone, we/I do hereby authorize the cultural exchange organization and its foreign partners to consent to any medical/surgical diagnosis, treatment, or hospital care that is advisable by and is rendered under the general supervision of a licensed physician or surgeon. We also agree that if the applicant becomes ill or incapacitated, we may take such actions considered necessary including securing medical treatment. We release the cultural exchange organization and its partners from all liability related to such actions.

CONDITIONS OF PARTICIPATION

The proposed program is a cultural and linguistic exchange. It is therefore necessary for participants to demonstrate curiosity and a willingness to adapt to the habits and customs of the family and the community that will welcome them. We count on each participant to act as an ambassador of their country and culture.
Hummingbird® relies on the information provided in the Travel Application when placing students and chaperones with host families. Students and chaperones are responsible for providing accurate and detailed information. Visitors should remember that these families are not paid, and must treat the families with courtesy and respect. After the students are matched, visiting students will not be allowed to request new host families. Students must be no older than 18 years of age at the time of the visit.*

I hereby authorize my student to participate in a homestay program organized by Hummingbird Student Tours. I am aware that all of the students participating in this program will be staying with host families, and that housing arrangements for my son/daughter/ward have been made by others in the destination country. I am aware that my student will be participating in activities which are not supervised by Hummingbird Student Tours, including but not limited to activities with the host family, host students, or other students. I grant to Hummingbird Student Tours and its authorized agents the right to take whatever action it deems necessary in regard to the health and safety of my student including, without limitation and at my own expense, obtaining medical treatment and, if necessary, transportation back home. I recognize that there may be risks associated with the program. On behalf of my student, I release Hummingbird Student Tours and its officers, directors, shareholders, employees, volunteers, partners, agents and representatives from any liability, including but not limited to liability for damage, illness, injury or death, expense, inconvenience, delay or other loss in connection with the tour. Hummingbird Student Tours does not own or operate any of the suppliers of services or accommodations for this trip, and, as a result, is not responsible for any negligent or willful act or failure to act of any supplier or of any third party over which Hummingbird Student Tours has no control.*
