2023 Sickle on the Green Golf Tournament Liability Release Waiver

THIS GOLF PARTICIPATION WAIVER MUST BE READ AND ACKNOWLEDGED IN ORDER TO PARTICIPATE IN THIS EVENT. By clicking the golf waiver box, proceeding through and paying for golf registration, participants acknowledged they have read and agreed to abide by the Sickle on the Green Golf Tournament Wavier.

In consideration of my participation in the foregoing, the undersigned acknowledges and agrees to the following:*

In consideration of the acceptance to the Sickle on the Green Golf Tournament, I waive all claims for myself, my heirs, and assignee against Sickle on the Green, Sickle on the Green sponsors, promoters, volunteers, Sickle Cell Partners of the Carolinas, and NorthStone Country Club and any related parent company from injury or illness which may result from my participation.I accept and take complete responsibility and ownership for my actions whether it's physical loss or property damage I cause to the entities of NorthStone Country Club, Sickle on the Green Golf Tournament, and/or Sickle Cell Partners of the Carolinas.I take complete responsibility for the amount of alcohol consumption I retain and any actions from the result of any alcohol consumption on my part.I give permission for the unrestricted use of my name, pictures, and video in any written account, broadcast, or telecast of this event for any legitimate purpose or future advertising.

Following the pronouncements above, I hereby declare the following:*

I am in proper condition to compete in this golf tournament and the risks associated therewith.I understand if the tournament is canceled because of any circumstances beyond the control of the golf committee, sponsors, Sickle Cell Partners of the Carolinas, Sickle on the Green, NorthStone Country Club and any related parent company including and not limited to hazardous weather conditions, a pandemic, health breakout or governmental ban. I understand my entry fee will not be returned.I understand in the event the Sickle on the Green Golf Tournament does not occur on its stated tournament date, Sickle on the Green tournament representatives will do their best to reschedule said event at a future date.I understand the Sickle on the Green tournament is a rain or shine event.

By signing below, I acknowledge that I have read the foregoing Liability Release Waiver and understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; that I have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed; that I give my voluntary consent in signing this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation.
