To receive ''Permanent" Jewelry from Link'd by Cherry, we require that you sign this consent form and liability waiver before we can complete your service. Please acknowledge each statement by checking the box.

Once you've completed the consent form you need to book and pay deposit before any appointment is confirmed.

1. I acknowledge that all sales and services provided are final once any chain is cut, and no return policy is applicable to any product purchased or service rendered.*

2. I acknowledge that Link'd by Cherry warrants that all products and/or services shall be completed in a workmanlike manner consistent with standards in the profession, or industry and shall be fit for ordinary use, of good quality, and with no material defects. In the event of a material defect, I must contact the Link'd by Cherry within 30 days of purchase, to coordinate necessary free repairs. However after 30 days you can coordinate to have your jewelry rewelded for a $20.00 fee.*

I acknowledge that my participation in receiving personalized welded jewelry services and procedures is voluntary and participating in obtaining permanent jewelry with knowledge of the procedures involved and hereby agree to accept and assume any and all risks known and unknown that may arise before, during and after the service. Including any jewelry that may turn or tarnish. I have been provided care instructions and after care kit.*

I Acknowledge
4. I hereby voluntarily release, forever discharge, and agree to indemnify, defend, and hold harmless Link'd by Cherry, their agents, owners, officers, volunteers, participants, and other persons or entities acting in any capacity on behalf of Link'd by Cherry from any and all claims, demands, or causes of action, which are in any way connected with services rendered or products sold by Link'd by Cherry.*
