  • Admission Requirements

  • Students are required to complete the following prior to final admission into the program:

    ·      Criminal Background Check

    ·      Proper Immunizations

    ·      PPD (TB) Test or Chest X-Ray

    ·      Drug Screen

    ·      Physical Assessment

  • Classroom/Lab Attendance Requirements

  •  Students are required to complete the 40 hours of classroom (ONLINE), 36 skills lab hours (IN PERSON), and 24 Clinical Hours (IN PERSON) to obtain their CNA Course Certificate. In the event that a student may not attend skills lab, they must contact the course instructor within 48 hours of the skills lab date to re-schedule a date that the instructor can perform a separate skills lab to catch you up with the rest of the class. If a student is tardy (10+ minutes late) to a skills lab date, they will be required to stay later that date to complete the required hours for the day. This will consist of the instructor going over the materials the students missed while late. If they are unable, they need to find a date with the instructor to make-up the time. Once a student has three tardies, this counts as a full absence. A student is allowed a maximum of three absences throughout the entire course. Students will be able to make up the three absences, once speaking to the instructor and finding a date suitable for the both of them. If a student goes over their allotted three absences, they will be suspended from the course, and will have the option to re-enroll in the next cohort, without refund.

    Students must complete full 100 hours of the course to obtain their CNA Course Certificate.

  • Clinical Attendance Requirements

  •  Students are required to complete the three days (24 hours) of clinicals to obtain their CNA Course Certificate. In the event that a student may not attend, they must contact the course instructor within 48 hours of the clinical date to re-schedule a date that the instructor can perform clinicals with the student and the Skilled Nursing Facility. If a student is tardy (10+ minutes late) to a clinical date, they will be required to stay later that date to complete the required 8 hours. If they are unable, they need to find a date with the instructor to make-up the time.

  • Code of Conduct

  • Requirements from the Georgia NATP Manual:

    A.  Students must wear the Winder CNA Nametag given at first course date. Students must also adhere to the dress code.

    B.   Students must interact respectfully and with integrity to all Instructors, Students, Coworkers, Professional Staff, Residents, etc.

    C.   No phone usage will be allowed in the Classroom, Lab, or Clinicals.

    D.   Disruptive behavior or inappropriate language such as profanity or inappropriate discussion of personal problems during any phase of training and testing is not permitted

    E.    Display professionalism and self-control at all times

    F.    Perform no task for which the trainee has not received training

    G.  Practice all safety precautions taught for each lab skill

    H.   Reporting appropriate information such as changes or problems in resident to instructor and/or supervisor; i.e. students must report all concerns immediately to the clinical instructor first, or a nursing home supervisor per nursing home policy

    I.     Follow HIPAA Guidelines at all times. HIPAA violations may result in dismissal from the program and may prevent placement on the Georgia Nurse Aide Registry

    J.    Respond to emergencies as needed

    K.  Cheating is prohibited and will result in the immediate dismissal from the nurse aide course

    L.    The removal of unauthorized program and/or testing documents is prohibited and will result in the immediate removal from the program. Eligibility for placement on the Certified Nursing Aide Registry will not be allowed

    M. Instructors/contractors must be paid according to the NATP service agreement with the program.

    Disciplinary Actions

    Failure to follow the Code of Conduct is immediate basis for dismissal form the program and/or loss of approval status with the State Contractor. The State Contractor reserves the right to remove approval for teaching/oversight status for program coordinators and/or instructors.

     Student Responsibilities
    Students are responsible for:

    • attending school on a regular basis and to be on time.
    • completing all academic work as assigned.
    • bringing appropriate materials to class.
    • obeying school/classroom rules.
    • respecting the rights of others.
    • their own actions.
    • using appropriate language.
    • respecting school property.
    • complying with requests of school employees.
    • dressing appropriately for school. 
       

    Student Rights
    Students have the right to:

    • an education.
    • attend a safe school.
    • physical safety and protection of personal property.
    • not be discriminated against.
    • privacy.
    • fair and just treatment by school employees. 
       

    Students are expected to follow the Dress Code, Attendance Policy, and Clinical Requirements. Any form of Cheating or Non-approved assistance on Homework, Quizzes, or exams will not be tolerated, and will be subject to student suspension with no refund given for any coursework. 

    Certain behaviors that are so serious in nature in terms of the disruptive effect upon the operation of the school may be deemed Exceptional Misconduct. Exceptional Misconduct includes, but is not limited to:

    • possession or sale of a controlled substance
    • profane or vulgar language directed to and in the presence of a staff member
    • arson
    • all fighting and/or assault and battery
    • harassment
    • defacing, misuse or destruction of property
    • defiance of school authority
    • possession of dangerous weapons
    • theft
    • At no point will Harassment, Intimidation or Bullying be tolerated at Winder CNA Training.

    If any of the code of conduct policies are broken, the student will be suspended without a course refund given. The student will also not be able to attend Winder CNA Training again.

  • Course Grading/Completion Rules

  • Students complete a variety of Homework/Participation, Skills Reviews, Quizzes, and Exams to test their knowledge of the coursework. The following grading scale will be used to determine if a student passes the course:

     

    Unit Homework – 10% of Grade

    Unit Quizzes – 30% of Grade

    Skills Reviews – 20% of Grade

    Unit Exams/Final Exam – 40% of Grade

     

    A Student must possess a overall course grade of 75% or higher to complete the course.

  • Dress Code

  • Students are permitted to wear casual slacks, jeans, and T-shirts, provided they are clean, in good repair, and do not contain any offensive language or pictures. Shorts of mid-thigh length or longer are permitted, except on lab days. Only closed-toe shoes are allowed in the classroom/lab. Students must wear their name tag while in the classroom or lab for proper identification.

    For either gender, Medical Scrubs are required to be worn on the days of Nursing Home Clinicals. Students must wear RED COLORED scrubs to clinicals along with their student nametag. Shoes must be comfortable, clean, in good repair and appropriate to the job functions and duties. Flip- flops, slippers, or open-toed shoes/sandals are NOT allowed in the patient-care setting. Hair must be neatly groomed and clean. Long hair may neither obstruct vision nor interfere in any way with the student's clinical performance. A hair restraint, i.e., hair net, may be required in certain settings. Also, hair color and style must be appropriate for the clinical work environment. Fingernails should be short, neat and clean (no longer than 1/4" past the finger Long fingernails are a safety hazard to self and others. Artificial nails are not permitted. No jewelry should be worn on clinical days.

  • Program Acknowledgement Form

  • You acknowledge that you will receive the blank, Alliant Health Solutions Nurse Aide Training Program Evaluation Form provided by Winder CNA Training. I understand that it is my obligation to fill out the evaluation form and mail to Alliant Health Solutions.

  • Accommodations for Students

  • Should any student need accommodations to complete the course, please contact the Instructor prior to the first day to setup any required accommodations.

     

  • Student Physical Assessment

  • The Student Physical Assessment will be completed one week before the course start date. More information regarding scheduling your Physical Assessment will be sent via email after registration.

  •  Students who chose to withdraw from the class may do so within three days before the course start date, up until, the fifth day of the course (1st Friday). If the student withdraws during the period stated previously, they will be refunded 100% of any monies paid to Winder CNA Training. Refunds will be provided within thirty (30) calendar days of receiving notice of cancellation. Students who cancel after the five-day period will not be entitled to any refund.

     

    Students must reserve their spot in the class by submitting a $99 non-refundable deposit when completing their registration form. This amount will be deducted from the total tuition amount. If the Student cancels their registration, the non-refundable deposit will be kept.

  • Waiver of Liability Agreement

  • I, {name}, acknowledge that my participation in the training program offered by Tri Delta Fye LLC DBA Winder CNA Training involves physical activity, including but not limited to clinical skills practice and potentially strenuous activities.


    In consideration of being permitted to participate in the program, I agree to assume all risks and responsibilities for any injury or medical condition that I may sustain or aggravate during the course of the program. I understand that the program may involve physical and mental challenges, and I am responsible for ensuring that I am in good health and suitable physical condition to participate.


    I hereby release, discharge, and hold harmless Tri Delta Fye LLC DBA Winder CNA Training, its instructors, employees, and any affiliated entities from any and all claims, demands, liabilities, actions, causes of action, costs, and expenses, whether at law or in equity, whether known or unknown, arising out of or connected with my participation in the training program.


    I understand that this Waiver of Liability Agreement is binding upon me, my heirs, executors, administrators, and assigns. I have read and voluntarily signed this agreement, and I acknowledge that I fully understand its contents.

  • Student Insurance Policy

  • As a student of Winder CNA Training, I acknowledge that I have chosen to enroll voluntarily in the program offered by Winder CNA Training, and I understand the inherent risks associated with participating in educational and clinical activities.

    I understand that while Winder CNA Training takes reasonable measures to ensure my safety during the course of my training, there may be circumstances beyond their control that could result in injury or harm. I hereby waive and release Winder CNA Training, its employees, instructors, agents, and representatives from any and all liability for any injuries, damages, or losses that may occur as a result of my participation in the program.

    I understand that it is recommended, though not required, that I obtain student liability insurance to protect myself in the event of any actions or incidents that may occur during my training or upon entering the healthcare industry as a licensed professional.

    I acknowledge that there are several companies that offer student liability insurance, including but not limited to:

    HPSO (Healthcare Providers Service Organization)
    NSO (Nurses Service Organization)
    I understand that it is my responsibility to research and obtain the appropriate insurance coverage to protect myself during my training and professional career in the healthcare industry.
