Pahrump Disability Outreach Program (PDOP) provides advocacy services as a community service to families participating in the PDOP Program.

I acknowledge by signing below that I will be afforded the opportunity to make all final decisions in all meetings and hearings and that the Pahrump Disability Outreach Program volunteer is strictly there to provide information, support, and guidance and is not a deciding factor in any of my decisions.

I acknowledge by signing below that I know that the volunteer from the Pahrump Disability Outreach Program is not an attorney, has no formal training, and is simply providing the best advice from years of experience, and I am requesting their assistance in the matter of my child.

I acknowledge by signing below that I waive, release, and forever discharge claims against the Pahrump Disability Outreach Program and any Pahrump Disability Outreach Program volunteer for any injuries, damages, losses, or claims, whether known or unknown, which arise during or as a result of their support of me and my child in school-related meetings and or hearings.

Iacknowledge by signing below that I agree to indemnify and hold the Pahrump Disability Outreach Program and the volunteers of Pahrump Disability Outreach Program harmless from all losses, liabilities, damages, costs or expenses (including but not limited to reasonable attorneys' fees and other litigations costs and expenses) incurred by any volunteer of Pahrump Disability Outreach program as a result of any claims or suits that may be brought against any of the volunteers to recover any losses, liabilities, costs, damages, or expenses which arise during or result from the participation by, or services supplied by any volunteer, except only to the extent that it is an intentional bad act by the part of the volunteer.

I understand that any recordings made of any meetings involving my child are done for liability protection and PDOP will ensure confidentiality and these recordings will not be shared without my permission.
