WAIVER AND RELEASE OF LIABILITY 

For all children listed as registrants.
It is understood and agreed that the undersigned shall not bring or be brought any action due to personal injury or property damage that might result from the undersigned's participation in any exercise, or workout as may be liberally interpreted, whether under the supervision of any instructor or by the undersigned's own direction. 

To restate, the undersigned agrees to accept full responsibility and to hold harmless Grit Fitness and/or any persons employed by or contracted by Grit Fitness. The undersigned must report immediately to the staff any affiliation as an exercise instructor, and any history of body injuries, defects, current pregnancy, knee problems, back or joint problems, etc…, so the said person may, at the discretion of the staff be removed promptly from the class or be allowed to remain in the class. Any failure to make public any of the aforementioned conditions shall be the sole responsibility of the undersigned. 

 

Model Release

Grit Fitness has the right to take my childs photo/video and post them to social media and use them in advertisements. 

Release for Parent /Guardian of Minor Child

I do attest that I am the parent or legal guardian of the above named minor child, and have legal authority to sign this release on his/her behalf. I have read and fully understand the contents of this release, and consent to the use of said photographs based on the contents thereof.
