  • Purpose

Positive Alliance, formerly known as Impac+NYC, is a 501(c)(3) organization dedicated to building relationships between HIV+ individuals, allies, and supportive resources. We're doing this survey to determine the needs of people throughout the community, and figure out how best we can serve your needs. Some of those needs are social and relate to human connection: what kinds of events would you like to see, to connect with the community? Other needs are more fundamental: do people have adequate resources? If not, how can we connect them with organizations that offer the support people need? All answers are optional, and we will not collect your personal data, link your identity to your answers, or share your identity or personal information with any third party. We will publish the results of our survey in July, and may share the overall results (not individual answers) with partner organizations that provide services related to HIV/AIDS treatment, support, and prevention.
  • Privacy Statement

Thank you for participating in the Positive Alliance Community Survey. No personally identifying information will be collected; therefore, your responses are unable to be tied to your identity. Your individual responses will not be available to anyone other than the data analyst creating a report(s).
  • Support comes from many different places.  We are interested in where community finds the most support from friends, family, coworkers and those that are not a part of the community.

  • HIV Affected

Positive Alliance is dedicated to the well-being of HIV+ individuals and to connecting people living with HIV (PLWH) and allies with one another and with opportunities to create positive change. We define HIV affected as anyone living with HIV/AIDS; people close to someone living with HIV/AIDS or anyone at risk of becoming infected with HIV.
  • You shared that you are a person living with HIV/AIDS.  The following questions are intended to understand your connection to care and to community.
