To help get your family dynamic piece completed and to complement the answers on your Information Sheet, please write Your Story in response to the following 10 questions.

1. Briefly describe your childhood including how you were parented. What did you enjoy most about your family and childhood? What did you like the least? Are you still close to your parents?

2. Were your parents believers? Did they take you to church and introduce you to Jesus? How old were you when you first learned of Christ and did you accept him as your Lord and Savior?

3. Describe your adolescence and later teen years?

4. Did you go to work after high school or attend college?

5. When did you meet your spouse? Were you both followers of Christ? How were the first few years of your marriage? Have they gotten better or worse? What have been your primary struggles?

6. When did you start your family? Did children draw you closer to your spouse or create new differences? What have been your greatest obstacles as parents? What would you consider to be your greatest reward as a parent?

7. Does your job require you to travel away from home for extended periods of time? What are your work hours? On average, how many hours a week are you available to interact with your children?

8. Describe your prodigal’s behavior that led you to this ministry. Has your prodigal affected the relationship with your other children? How have they been impacted by these struggles?

9. What do you hope to accomplish in this ministry? What does a win look like?

10. What are your fears? What are your greatest obstacles?
