Immunizations & Vision and Hearing Screenings
The student's immunization record is on file at the public/private school the student attends. All immunizations are current. All necessary Vision and Hearing Screenings as required by the Special Senses and Communication Disorders Act are current and on file at the students school.

At this time, Aim and Focus Karate does not require staff members to have specific immunizations or vacinations.

Registration Fee

There is a $50 non-refundable Registration Fee required before your enrollment will be accepted. There are also fees associated with the martial arts trainings (test fees, sparring gear, national association membership, etc.). These fees only apply when you approve of the purchase / clinic / belt test / etc.

All-Day-Out charge: $30

Tang Soo Do Mi Guk Kwan annual membership fee:

All students must join this national martial arts organization. The annual membership rates are: $30 First family member, $20 Additional family members, $40 Black Belt Memberships

NOTE: Aim and Focus Karate pays the annual fee on behalf of our students.

I am giving permission for the student listed on this form to participate in the 2024-25 Aim and Focus Karate After-School Program. I realize the staff and instructors will do everything in their power to protect my child when in their care. I will not hold them responsible and waive all claims against the Aim and Focus Karate School Inc. for any accidents that may occur when the student is in their care. I also acknowledge that Aim and Focus Karate Inc. does not provide medical insurance coverage for accidents.
