I understand that the tattooing process involves certain risks, including but not limited to infection, allergic reactions, and dissatisfaction with the final result. I have been provided with information regarding the proper aftercare procedures for my tattoo and agree to follow them to ensure proper healing.

I confirm that I am not under the influence of alcohol or drugs at the time of receiving this tattoo, and I am of sound mind to make this decision

I understand that the tattoo design discussed with the artist is final, and any changes made during the process are subject to the artist's discretion and may incur extra cost. Once a tattoo is started ,regardless of completion, in the event a customer stops the tattoo process, the full price is collected.

I release Sofia Gomez from any liability for any damages, losses, or injuries that may occur as a result of the tattooing process.
