Authorization to Repair: The Estimate to Repair includes parts, labor, and necessary diagnoses. If, upon further inspection, additional parts or repairs are required, I authorize Unique Auto Body to contact either myself or my insurance company for approval and any additional funds needed to complete the repair.
I authorize the repair work to be performed by Unique Auto Body, including any subcontractor(s) hired to complete portions of repairs, along with the necessary materials and grant Unique Auto Body & subcontractor(s) employees’ permission to operate my vehicle for the purpose of testing, inspection, quality verification, or transport between facilities as needed to complete the repair process. I further grant permission for my vehicle to be moved to another Unique Auto Body location for scheduling or load-balancing purposes.
In the event my insurance company declares my vehicle a total loss and I choose to keep the vehicle, I agree to pay all teardown, disassembly, and inspection charges incurred prior to that determination. I understand that any damaged components that were removed for estimating purposes will not be reassembled onto a vehicle that has been classified as a total loss.
I understand that Unique Auto Body is not responsible for loss or damage to my vehicle resulting from fire, theft, accidents, weather events, or any other causes beyond their control (“Acts of God”).
Vehicles Equipped with Service Mode: For vehicles equipped with a “Service Mode” or similar function, I authorize Unique Auto Body to activate and utilize such mode for diagnostic, calibration, and repair purposes. I understand that while my vehicle is in service mode, all mobile access and remote functionality may be temporarily disabled.
Electronic Communication Consent: I authorize Unique Auto Body to communicate with me via telephone, email, and text message regarding the status of my vehicle, repair approvals, or related account matters. I understand that these communications may include automated notifications and that message and data rates may apply depending on my carrier plan.
Storage Fees & Lien Sale: Customer authorizes & acknowledges that if the vehicle is not picked up within seventy-two (72) hours after Unique Auto Body has notified the Customer of completed repairs, the repair facility may charge storage fees at rates that are ordinary & customary for the area, but not to exceed $65.00 per day. In addition to any, and all, available legal & equitable remedies, Unique Auto Body may, in accordance with applicable state law, begin lien sale proceedings.
Payment Terms: Payment in full is due upon completion of repairs and prior to the release of the vehicle. Unique Auto Body accepts cash, credit card, or certified funds unless other arrangements are approved in advance. Any balance not paid when due is subject to an interest charge of 1.5% per month (18% annual percentage rate) or the maximum rate permitted by law. In the event of a returned check or declined payment, additional fees may apply. Unique Auto Body reserves the right to retain possession of the vehicle until payment is made in full. An express mechanic’s lien is hereby acknowledged on the vehicle to secure payment for the amount of the authorized repairs. My signature authorizes Unique Auto Body to endorse and deposit any insurance drafts or checks made payable to me for the purpose of these repairs.
