Jenkins Youth and Family Village Summer Program 2024 Enrollment Form

 

Summer Program Dates & Time:

June 10, 2024 – July 26, 2024 

8:00am – 4:00pm 

The Summer program at JYFV is FREE of Charge. However, we need and appreciate parents who help us by making monetary and/or food donations; by volunteering to help with activities, field trips, etc.; and by helping to keep the environment clean and safe for the children.

Registration Form is required for the Jenkins Youth and Family Village Summer Camp

Community, Partners, Neighbors and/or Volunteer Agreement

Parents/Guardians on behalf of themselves, their spouse and their children hereby agree that they will not hold liable Jenkins Youth and Family Village, its board members, directors, officers, sponsoring agencies, sponsors and affiliates, contracted instructors/trainers for any and all liabilities including loss, injury or damage due to fire, theft, accidents, or any cause whatsoever that may arise or occur in connection with services and activities held at Jenkins Youth and Family Village while participating in an event or activity as part of the program. Further, the signee agrees to hold harmless Jenkins Youth and Family Village and its affiliates from any and all claims, liabilities, losses, and cost (including reasonable attorney’s fees) as a result of participating in the program events and sponsored activities. Parents are encouraged to attend programs, meetings and help with fundraising, make monetary and/or food donations; volunteering to help with activities, field trips, etc. and help to keep and maintain the environment at the Center clean and safe.

I hereby grant permission to the rights of image, likeness, and sound of my/child(ren) voice as recorded on audio or video tape without payment or any other consideration. I understand that my child(ren) image may be edited, copied, exhibited, published, or distributed and waive the right to unspicy or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my child(ren) image or recording. I also understand that is material may be used in diverse educational settings within an unrestricted geographic area.

Photographic, audio or video recordings may be used for the following purposes:

• Social media/Advertisement features further the work at Jenkins Youth and Family Village
• Conference presentations
• Educational presentations or courses
• Informational presentations
• On-line educational courses
• Educational videos

By signing this release, I understand this permission signifies that photographic or video recording of me/my child(ren) may be electronically displayed via the internet or in the public educational setting.

I will be consulted about the use of the photographs or video recordings for any purpose other than listed above.

There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.

This release applies to photographic, audio or video recordings collected as part of the sessions listed on this document only.

By signing this form, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release all claims against any person or organization utilizing this matter for educational purposes.
