SDC Student Certificate Enquiry Form

Dear student, thank you for taking time to fill in the form. Your request will be processed internally by our facilitating department and escalated to quality assurance if required.
PLEASE NOTE that outstanding certificates may be due to the following reasons:
- The student is unreachable due to their contact details that recently changed. Please provide updated and correct contact details when submitting your query.
- The student may have missed a call from SDC to inform them about the collection of their certificate. Please note that students are required to collect their certificates from the SDC campus physically.
- The student's POE (portfolio of evidence) is incomplete and has not been handed over to the exit unit. The facilitation department must finalise this with the student.
- The student's file is completed and handed to the exit department, and may be in one of the following exit phases which generally takes 6-months for completion:
  1. Phase 1: Undergoing internal moderation at SDC
  2. Phase 2: SDC is awaiting external verification from the SETA
  3. Phase 3: SDC is awaiting for actual delivery of the certificate
This form is for students who have completed their learnership and have NOT received their certificate.
