This award is for the NHS Categories ONLY. Please fill out this form to nominate an NHS Employee or NHS Trust for an award.

GUIDELINES

• You may enter 1 project per category and can enter as many categories as you wish.
• Entrants must have obtained the permission of the nominated trust or employee to enter their work into the awards.
• All projects must have been delivered since 2021.
• The decision of the judges is final, and no correspondence will be entered into. 
• Entries received after the deadline (17th October) will not be accepted. 
• Entries may be moved to a different category at the discretion of the judges. 
• By entering the awards, you agree to all information to be published in our annual report.

CRITERIA

In addition to the below questions, the judges will be looking for the following throughout your responses.

• The person or project shows innovation, imagination and ambition to engage staff and overcome project challenges
• Evidence of a good clear strategy for the project or policy 
• Evidence of good management 
• Evidence that the strategy was clearly communicated to staff and other stakeholders
• Value for money (if applicable)
• Supporting evidence that the project/policy has made a difference to the organisation 
• Evidence it has achieved or is well on the way to achieving what it set out to do
