I, the participant or the parent of the participant, understand laser tag is an active participation sport that may expose the participants, bystanders and property to many types of injury or damage. In consideration of this potental injury risk, I give my informed and voluntary consent to myself and/or my minor children, listed above to participate in the laser tag activity. Any laser tag participation is at my own risk.

I agree to indeminfy and hold harmless Tom' Laser Tag, its officers, agents, or employees liable or pursue legal action against them. This waiver absolves Tom' Laser Tag of any duty for injuries sustained before to, during, or after the related laser tag activity. By signing this agreement, I agree to hold the company completely harmless for any injuries sustained, regardless of the cause or circumstances.
