    Introduction Course Registration Form
  • It is so great that you are keen to have a go at underwater hockey, the world's most breathtaking sport!  You will learn some great new skills in the water.

Please complete this form for each person you are registering to try Underwater Hockey.

This form should be completed by the player, or if the player is under 18 years it should be completed by a parent or guardian.

  • Player Contact details:

(If the player is under 18, enter the parent's contact details if you prefer)

  • Emergency Contact details

  • Water Competency and Safety

To ensure that this course is safe for all participants, players need to be able to swim competently before trying underwater hockey.
  • Participants do not need to have snorkelled before this course. We can teach them but it is helpful to know if the player has had prior experience.

  • If the player is under 15 years of age, parents are required to stay at the pool area for the duration of each session, and are expected to keep an eye on their own child.  Parents do not need to be in the water with their child, but they are welcome to do so if they wish.

  • While underwater hockey is a non-contact sport, there is a chance of accidental injury, usually from being kicked by another swimmer or hit with a stick or puck.  We take precautions to minimise this risk. Read about this on our website here.

  • Underwater sports involve depths and pressures that may affect a person’s sinuses and ears. Participants should be feeling fit and well when undertaking these sports. Please note that certain medical conditions may preclude a person from participating. We will provide a Medical Checklist to be completed before participating.

  • Does you have any special needs or is there any other information you would like to give us about the player which could be helpful to our instructors?  This is your chance to tell us anything you like, or ask a question!

  • Photos & video

We may take photographs or video footage of the participants in order to promote underwater hockey on our website, on social media or printed materials such as brochures. We avoid using images of children wearing only bathers when out of the pool. In the pool, children are usually wearing only swimwear, however images used will be groups of children (not single children) and will be either playing underwater hockey or posing for group photos (with torsos underwater). We avoid using close up images of any individual child's torso. If we wish to use an image that is outside of these guidelines, we will contact a Parent to seek express permission.
  • Gear

We will provide all the gear specific to underwater hockey such as gloves, caps, sticks. We ask that you supply your own snorkel, mask and fins.
  • Costs

  • A single course fee of $80 for adults (or $50 for children under 18) is payable to SA Underwater Hockey Club prior to the course commencing. This includes pool entry fees (so do not pay at the desk).

You need to supply your own snorkel, mask and fins for each participant, as indicated on the course website page.

We are a small not-for-profit club so we don't have online payment facilities. We will contact you to arrange payment via electronic transfer to our bank once your registration has been processed. Thank you!

  • When you submit your form, you will be sent an email with your responses.

We will review your registration soon and send you the details about your first come and try session.  Thank you!
