  • INSTRUCTIONS :

    Step 1: Fill out Questions below, and the 'NEXT' button will appear.

    Step 2. Press the 'NEXT' button, and the calendar section will appear.

    Step 3. Calendar Section: Select and Submit Calendar Day and Time by choosing day, time, and other information below.

    Step 3. Final Step: Press the 'SUBMIT APPLICATION' button after completing the calendar information at the bottom of the form below.

    Step 4. If submission fails, follow the instructions at the end of the form below. If you have any problems, reach out to us at support@guymonroe.com.

  • A [NEXT] button for Page #2 SCHEDULING will appear when Question 8 is filled out.  Page #2 SCHEDULING page permits you to select a day and time for your FREE Lesson with Guy Monroe.

  • Excellent! Now that you have completed choosing your Day and Time, your ready for the FINAL step.  PRESS the 'SUBMIT APPLICATION' BUTTON BELOW to SUBMIT your completed Application Form for review. 

  • NEED HELP? : DID SUBMISSION FAIL? : If nothing happened when you pressed the SUBMIT button, did you leave any fields blank? Make sure every field in the form contains content.  If you PRESS 'Submit Application' with EMPTY FIELDS,  NOTHING will occur.   This is because ALL * REQUIRED FIELDS must be filled in for submit form to be accepted.  If submission failure occurs, Follow the following Steps: 

    (Step 1.) Scroll through entire questionaire,

    (Step 2.) FIND AND FILL EMPTY FIELDS.  

    (Step 3.) PRESS SUBMIT. 

    (Step 4.) SUCCESS PAGE : When ALL fields are filled out correctly, the screen will refresh upon submission and you will be taken to a Congratulations Page!
