The undersigned acknowledges receipt of information regarding the breed, physical and personality characteristics of the animal being fostered, or foster to adopt, or adopted. The undersigned also acknowledges the risks and responsibilities associated with owning a dog or cat, and freely accepts them, and waives any rights to make a claim the rescue in the event that the animal bites, or causes injury, destroys property or succumbs to health problems that existed before or at the time of adoption. Sweet Angel Dog Rescue does not make any guarantees on the health or age of the animal. If the animal needs veterinary care while applicant is fostering the animal, applicant agrees to contact Sweet Angel Dog Rescue immediately before taking the animal to the vet clinic. Sweet Angel Dog Rescue will be responsible for any veterinary needs the animals has while in foster care, and any other usual, reasonable costs. Foster may purchase items for the animal, but if purchased without prior written consent from the rescue, then foster may bear the cost. If foster takes the animal to an unauthorized vet clinic without prior written authorization from Sweet Angel Dog Rescue, foster may bear the cost for any treatment., diagnostics, exams, etc. If the animal bites someone or another animal while in foster care, the foster must report the bite immediately to Sweet Angel Dog Rescue including the circumstances, date, nature or extent of injury, name and contact number of person bit, name of witnesses to the incident. Failure to timely report the incident may result in the foster being legally liable for any injury or damages caused by the animal. Applicant agrees to take the animal to veterinary appointments, trainings, or vaccination clinics as needed and scheduled by the rescue (with foster’s input on availability). Foster agrees to provide reasonable and responsible post-surgery or treatment care for the animal. The undersigned also agrees that, in the event s/he can no longer provide a safe home for the animal being adopted or fostered, s/he will consult with Sweet Angel Dog Rescue and return the animal if requested to do so. Under no circumstances shall the animal be surrendered to a shelter facility. If Sweet Angel Dog Rescue learns of neglect or abuse of the animal by applicant Sweet Angel Dog Rescue has the right to reclaim possession of the animal with 24 hours verbal notice to applicant, and applicant agrees to allow Sweet Angel Dog Rescue to pick up the dog. Applicant agrees to allow Sweet Angel Dog Rescue to see the animal in person, and surrender the animal upon request in that situation. The undersigned understands and acknowledges that the completion of this application does not guarantee that the applicant is approved for the animal, and Sweet Angel Dog Rescue will conduct a further determination process to assess eligibility and suitability of placement of animal with the applicant. Sweet Angel Dog Rescue reserves the right to decline an application, or cancel a foster or foster to adopt agreement if it determines that proceeding with the applicant is not in the best interests of the animal. The undersigned agrees to return the animal within 24 hours to rescue if that determination is made. It is our goal to place dogs in loving foster homes and if the foster family decides to adopt and they have been providing quality care then the foster gets first pick to adopt. It is in the best interest of the dog to be placed in a loving home that wants the dog to become part of the family. If the home is foster only, the rescue will look for a permanent home. We require a 3 month commitment unless there is a safety issue. In this case we will work as quickly as possible to find a new home. We do not have a facility, or standby fosters. __ _______ SPAY/NEUTER AGREEMENT: The undersigned understands that the animal that is being adopted may still be intact, due to age of the animal or health and inability to have surgery at the time of adoption. The rescue is, by law, required to have the animal spayed or neutered prior to adoption where appropriate, but may allow an animal to be adopted pending an agreement with the adopter to have the animal spayed or neutered at a later date. The undersigned agrees to have the animal spayed or neutered at the earliest opportunity when the animal is healthy, but no later than the first birthday of the animal when the animal is less than one year old at the time of adoption and healthy. The undersigned agrees to exercise extraordinary care to prevent the animal from becoming pregnant or mating with another dog because such would be incredibly irresponsible and in breach of this agreement. The undersigned will provide the rescue with a receipt as proof of the sterilization surgery promptly upon receipt. Date: __ Date _______________ Signature of applicant: Signature
