By signing this waiver, I acknowledge that I am voluntarily participating in physical activities offered by Bandz N Booty, including but not limited to dance fitness classes, pop-ups, and events.
ASSUMPTION OF RISK
I understand that participation in any physical activity involves inherent risks of injury, including but not limited to muscle strains, sprains, falls, and other possible medical issues. I voluntarily assume all risks associated with my participation.

HEALTH STATUS
I affirm that I am in good physical condition and do not suffer from any condition that would prevent or limit my participation in this activity. I have consulted with my physician, or I am participating at my own risk.

RELEASE OF LIABILITY
In consideration of my participation, I hereby release, waive, discharge, and hold harmless Bandz N Booty, its instructors, contractors, volunteers, and event partners from any and all claims, liabilities, demands, damages, or causes of action arising out of or related to any loss, injury, or damages sustained during or as a result of my participation.

CANCELLATION POLICY
I understand and agree to the cancellation policy: Cancellations must be made 12 hours or more before class time to receive a refund or class credit. Cancellations made less than 12 hours before class will be non-refundable.
