  • INFORMED CONSENT, WAIVER, AND RELEASE FOR CREEKSIDE CANINE, LLC

    I recognize that there are inherent risks of illness and/or injury when dogs are allowed to co-mingle and engage in canine pack behavior. Such risks include but are not limited to problems arising from rough play, chasing, ingesting found objects and kennel cough. I understand and agree that any problem or injury sustained while my dog is at Creekside Canine, LLC will be treated as deemed best by Creekside Canine, LLC staff. In the event of a serious medical problem, Creekside Canine, LLC's may consult veterinary

    services at the expense of the owner.

    I, the owner, agree I am solely responsible for any and all acts and behaviors of my dog(s) while in the care of Creekside Canine, LLC, and as such I shall pay any and all such expenses incurred promptly. I understand that I am responsible for any harm caused by my dog(s) while attending Creekside Canine, LLC. I shall indemnify Creekside Canine, LLC against any and all claims made against, or losses or damages of any kind suffered by Creekside Canine, LLC or its members as a result of my failure to inform Creekside Canine, LLC of any medical conditions or behavioral issues or tendencies my dog(s) may have. I agree that in bringing my dog(s) to Creekside Canine, LLC, the facility has relied on my representations that my dog(s) are in good health and that they do not pose a health or behavioral threat to people or other dogs. Should my dog(s) exhibit inappropriate behavior which is deemed to compromise their own or other dogs' safety or that of the staff, I understand that my dog(s) will be safely confined as necessary until the owner or their authorized representative can remove the dog from the premises. I agree that Creekside Canine, LLC will not be liable for problems, damage or injury caused by or to my dog(s I understand that Creekside Canine, LLC is fully insured and I release Creekside Canine, LLC, Kate Dean, or any members of said Creekside Canine, LLC from any liability arising from my dog(s) attendance and participation at Creekside Canine, LLC and further agree to Indemnify Creekside Canine, LLC, Kate Dean, or any members of said Creekside Canine, LLC for claims made against them arising out of my dog(s) stay at Creekside Canine, LLC. I acknowledge that my dog(s) may be placed in a kennel for 'time outs' for aggressive or unwanted behavior in addition to meal times and overnight sleeping. I also understand that when dogs engage in normal pack play, nicks and scratches often occur. While my dog(s) will be checked and attended to daily by Creekside

  • Canine, LLC staff, it is possible for minor injuries to be inadvertently missed. If a serious injury is sustained, I, or my authorized representative will be notified as soon as possible, along with a veterinary service. I understand and agree that any problem or injury that arises with my dog(s) will be treated as deemed best by Creekside Canine, LLC. I understand that Creekside Canine, LLC provides cots, blankets, and bowls for all guests and it is at my own risk if I choose to bring any items, including leashes, toys, beds, bowls, etc. I agree that if I choose to bring any personal belongings for my dog(s) during their visit, those items may be damaged, destroyed or misplaced, and Creekside Canine, LLC will not be held responsible. I understand that my dog(s) may be photographed and/or recorded while at Creekside Canine, LLC. All such images will be the sole property of Creekside Canine, LLC and may be used to advertise, promote or publicize the facility. I understand that if my dog(s) are left at Creekside Canine, LLC for a period of 72 hours beyond the agreed length of stay, without contact from myself or my authorized representative that the animals will be judged to be abandoned and the necessary steps will be taken to place them with the proper authorities. 

    Training Contract: I understand I must be honest about my dog's human-directed aggression, no matter how mild it may seem. I certify that my dog does not display any human aggression like growling, snarling, baring teeth, lunging, charging, snapping, mouthing, biting or attempting to bite, staring, raised hackles, or rigid body posture directed toward humans) and/or severe separation anxiety. Dogs that display these behaviors must get an Evaluation. Lunging is not considered human-aggressive if not directed toward a human.

    Human aggression: unless approved through an Evaluation, dogs who growl, snarl, bare teeth, lunge, charge, snap, bite or attempt to bite, raise hackles, etc. directed toward trainers or other humans will be removed from the program and sent home immediately at the owner's expense. Training may not be transferred to another dog. I understand there will be no refunds for board & train packages when any "human-directed" aggression or severe separation anxiety is displayed during the program. 

    I realize that while training is often effective dogs are individuals that react to their environment in their own ways. Therefore, Katie Beth Dean is not responsible or liable for any incidents involving dog and/or owner before, during or after training has taken place. I understand that training is a continuous process throughout my dogs life and that I will need to maintain my dogs training once training is complete.

    I understand that the training will result in the dog responding to the Trainer and that the dog may not initially respond as reliably to the Owner. To ensure Dog's response to Owner, Owner should work to understand and apply the basic principles of training as described and demonstrated in the Transfer Session, and in the Training Handbook sent home. The owner should work to use the cues (commands) supplied by Trainer and to practice with the Dog daily (30-40 minutes a day, 6 days a week) To integrate training into daily life (as described by Trainer and Training Handbook) SO as to support behavior goals (not inadvertently punishing the good behavior and rewarding the bad, To follow training and management advice as given by Trainer. Training Payment is Not Refundable. No refunds are available on any training packages. Trainer will be available for phone calls and emails for a 2-month period after the to help owner maintain training.

    I understand that some training methods may include physical correction. As an owner, I know I can discuss possible alternative training methods. I understand that any adverse corrective tools will only be used after a discussion between the owner and Katie Beth Dean. By signing here, I consent to the use of corrective tools (prong, e-collar, etc) if needed. (Does not apply to Puppy Preschool as no dogs under 6 months would be recommended the use of aversive tools.)

     I also understand any nutritional or husbandry advice given by KB. Dean is just that - advice. If I suspect any serious medical issues, I should contact my Veterinarian immediately.

    I understand and acknowledge that payment in full is expected at drop off for expected services and Services that are not able to be rendered due to the behavior of a dog or situation change of the owner still requires payment for dates booked. Also, any deferment of payment, credit or refunds is not a guarantee and only available at the discretion of the owner KB Dean/Creekside Canine, LLC.
