Waiver and Release:
- During the school year, the Hanuman Mandir occasionally has the opportunity to use photos to promote the Sunday School programs and other school activities. Uses might include a display board, temple newsletter, temple website, press releases, etc. No names will be used on the website or in any photograph. I give the Sunday School at the Hanuman Mandir permission to include my child(ren) in photos used for informational or promotional purposes.
- As legal guardian of the minor who participates in Sunday school programs, I accept unto myself all responsibility and all liability for any injury or loss or damage that occurs to me and/or to the minor as a result of the minor’s participation in Sunday school programs.
- As a precautionary measure, if my child has a fever or has vomited within 48 hours, I will not send them to Sunday School.
- I understand that classes may move to remote platform in case of COVID or other related concerns, with short notice. All notices will be sent via email.
- I understand that the teachers at the Sunday School programs are not trained to handle students requiring an IEP. Under special circumstances, parents may be requested to accompany the child during the entire session of class meeting, or denied admission.
- I accept that students will be placed in classrooms based on strict age-related policies. Minimum age of acceptance is 5 years old as of Sep 1, 2025 without any exceptions.
- I understand that the Sunday School will not be able to administer any medication during the duration of the class meeting.
- I understand that I may be requested to volunteer when the need arises to help support the Sunday School.
REFUND POLICY: All fees are a donation to the Hanuman Mandir and therefore non-refundable. However, if a refund is requested in writing by Oct 1, 2025, a 75% refund will be processed by the Hanuman Mandir.

In consideration of your accepting my child’s participation in the above program, I hereby, for myself and my child, waive and release The Hanuman Mandir of Greater Chicago, and the teachers/ volunteers participating in the program, from all liability or claims arising from any injury to myself, my child or my property.

Fee/Donation information: $221 per child. Please make checks payable to the Hanuman Mandir.

Note: All fees go directly to the Hanuman Mandir of Greater Chicago. Instructors do not receive remuneration from the temple. The Hanuman Mandir of Greater Chicago is a charitable organization under IRC 501(c)(3). Therefore, donations and other fees, to the extent allowable under the Code, are deductible for tax purposes.
